Permanency Outcome 2: The continuity <strong>of</strong> family relationships <strong>and</strong> connections is preserved<br />

for children<br />

Item 11: Proximity <strong>of</strong> foster care placement<br />

Goal: To determine whether concerted efforts are being made to ensure that the child’s foster care placement is close<br />

enough to the parent(s) to facilitate face-to-face contact between the child <strong>and</strong> the parent(s) while the child is in foster<br />

care.<br />

To reach this goal, the State will ensure that the child’s current placement is close enough to his or her parents or other<br />

potential permanent caregivers to facilitate frequent face-to-face contact between the child <strong>and</strong> the parents/permanent<br />

caregivers while the child is in foster care (unless the placement is based on the child’s needs <strong>and</strong> intended to ensure that<br />

the child’s case plan goals are achieved <strong>and</strong> are in the best interest <strong>of</strong> the child) <strong>and</strong> that these efforts are appropriately<br />

documented in UNITY in a minimum <strong>of</strong> 90% <strong>of</strong> cases. Results from the 2009 CFSR, as listed in Table 20, demonstrate<br />

that Nevada is effectively meeting this goal <strong>and</strong> that this is a strength for Nevada child welfare agencies.<br />

Item 12: Placement with siblings<br />

Goal: To determine if concerted efforts are being made to ensure that siblings in foster care are placed together unless a<br />

separation was necessary to meet the needs <strong>of</strong> one <strong>of</strong> the siblings.<br />

To meet this goal, the State will ensure that the child <strong>and</strong> his or her siblings who are also in care are in the same<br />

placement (unless there is a valid reason not to do so, such as it is not in the best interest <strong>of</strong> the child) <strong>and</strong> that this<br />

information is appropriately documented in UNITY in a minimum <strong>of</strong> 90% <strong>of</strong> cases. Table 21 shows that this is another<br />

area <strong>of</strong> strength for Nevada. 2009 CFSR results demonstrate that 100% <strong>of</strong> cases reviewed show that each agency is<br />

exceeding this goal.<br />
