8. One hundred twenty-two (122) of one hundred twenty-eight (128) matrix spike recoveries
were outside acceptance criteria. A copy of the Sample Discrepancy Report (SDR) has
been enclosed.
9. Sixty-three (63) of sixty-four (64) blank spike recoveries were outside acceptance criteria.

10. The method blank contained the common laboratory contaminant Bis (2-Ethylhexyl)
phthalate at a level less than the CRQL.
11. Internal standard area criteria were not met for sample JllKK8 MSD. The analysis of
associated matrix spike sample fulfills the reanalysis requirement.
12. Manual integrations are performed according to SOP QA-125 to produce quality data with
the utmost integrity. All manual integrations are required to be technically valid and
properly documented. Appropriate technical flags are defined in the Glossary ("Technical
Flags For Manual Integration").
13. LvLI is NELAP accredited by the state of Pennsylvania and holds over 20 additional state
accreditations. For a complete listing of accrediting authorities and the corresponding
