It appeared that some of the breakdowns in the events occurred where there was ambiguity in framework that governed relationships and responsibilities. A third party was engaged to do this evaluation, but when they were unable to complete it, an internal review was done, with another third party asked to review that. The recommendations are being sent to the business leaders in the organization for their response and recommendations. The broader leadership of the organization will also review them and assess what further response is needed. 3.
