These forms are used to monitor program implementation, dosageand fidelity by documenting the type of program, the school where the program isimplemented, the grade level(s) of the youth participating, the number of youthcompleting the pre-test and post-test, the number of lessons implemented and thefrequency of program sessions.Action Plan AnalysisDuring the first project year, project staff followed the Action Plan submitted in the grantproposal.
