The Elder Equity Consortium (NAPCA, NAPPM, NCBA,<br />
NICOA, and SAGE) brings a thorough knowledge of the unique<br />
needs and concerns of each population they serve. Their<br />
diverse perspectives offer an inclusive vision of both the<br />
strengths and disparities of each community, while creating<br />
solutions and advocacy focused on elders. This interactive<br />
session will provide an opportunity to share insights and<br />
techniques about effective outreach and meaningful involvement<br />

Learn about how the Chicago Network for LGBT Aging has<br />
served as a vital hub for providers and community members<br />
concerned about the needs of this underserved and growing<br />
population. As a volunteer-led organization, the Network has<br />
successfully fostered collaborations and networking among a<br />
diverse group of stakeholders by leveraging technology and<br />
affiliate resources.<br />

Hear how a California health plan has partnered with community-based<br />
organizations in the aging and disability networks to<br />
successfully transition Medicaid and dual eligible beneficaries<br />
from LTC facilities to the community. The health plan will share<br />
its efforts with California’s Money Follows the Person program,<br />
as well as a pilot program. The panel will share challenges, and<br />
provide real-life examples of strategies and interventions used<br />
to overcome the challenges.<br />

for service providers. Stakeholders, agency leadership and staff,<br />
persons served, and funders want to know how dollars are being<br />
used, with what effect and how efficiently. This session will<br />
cover one agency’s effort to implement a comprehensive<br />
outcomes management system based on the CARF model.<br />

This session is part of a full-day program presented by ASA’s<br />
Healthcare and Aging Network (HAN).<br />
The number of older Americans will nearly double over the next<br />
25 years. This growth presents unique opportunities and challenges<br />
for community-based organizations (CBOs). At the same<br />
time, there is growing recognition that the supports CBOs provide<br />
are critical in helping older adults remain healthy. This session will<br />
examine the vital need for partnerships between CBOs and<br />
healthcare, as well as provide some examples of successful<br />
strategies for developing and sustaining these relationships.<br />
