Finding services and supports for people with self-care needs<br />

can be unnerving. However, states are transforming their<br />

fragmented LTSS access functions into single statewide Aging<br />

and Disability Resource Center/No Wrong Door systems to<br />

work better for consumers and their families. The AARP<br />

Scorecard project, funded by The SCAN Foundation and the<br />

Commonwealth Fund and in collaboration with the U.S. Administration<br />

for Community Living, researched promising practices<br />

to speed up the pace of change.<br />

MO111 HECM and Housing: An Aging-in-Place<br />

Tool With Multiple Applications<br />

The home equity conversion mortgage (HCEM) is a widely<br />

misunderstood and complex product that has been underutilized<br />

by the senior population because of the complexity and past<br />

abuses. Recent changes attempt to bring the product more<br />

mainstream, and recent research indicates that a HECM added to<br />

a retirement plan can increase the likelihood of NOT outliving<br />

retirement funds. Often considered a product of last resort, this<br />

perception is changing due to recent changes, research and media.<br />

MO113 Leave Me Alone: The Live-Alone Projects<br />

The numbers of people who live alone in the community and have<br />

Alzheimer’s disease and related disorders (ADRD) are growing at<br />

staggering rates. Services for this population are scarce and<br />

interventions are often unsuccessful. This workshop will report on<br />

three projects in California that identify, assess and connect<br />

people living alone with ADRD to programs and services to help<br />

them continue to live at home independently and safely.<br />

to Reduce Falls Among Older Adults<br />

The National Council on Aging’s National Falls Prevention<br />

Resource Center and the Administration for Community Living<br />

will provide updates on national efforts to prevent falls among<br />

older adults, including grants to states and Tribal organizations<br />

to implement evidence-based falls prevention programs.<br />

Presenters will highlight best practices, lessons learned, outcomes<br />

and sustainability strategies.<br />

MO117 The Little Home That Could: A Traditional<br />

Home Person-Centered Success Story<br />

Since 2013, the state of Kansas has been incentivizing personcentered<br />

care through a pay-for-performance program entitled<br />

PEAK (Promoting Excellent Alternatives in Kansas nursing<br />

homes). This program provides structure and a shared definition<br />

as well as a rigorous evaluation system. The majority of early<br />

adopters have been nonprofit and CCRCs but for one exception,<br />

Medicalodges Columbus. Learn how this very traditional<br />

for-profit, corporate-operated home has broken the mold with<br />

some dramatic successes.<br />
