which improves services for elder Medicare beneficiaries with<br />

services, and local monitoring and management of the area’s<br />
quality and supply of services.<br />

The retelling of stories keeps religions alive and transmits<br />
spiritual values and precepts to those who follow a particular<br />
tradition. Adherents of these traditions must pass on their<br />
beliefs and values to others and to the wider tradition. By leaving<br />
a spiritual legacy, persons share values and beliefs with other<br />
generations and find meaning in their own living and dying.<br />
Ethical wills can be an effective tool for leaving this legacy.<br />

positive life-changing results for individuals as they age. AT Act<br />
<strong>Program</strong>s have created models of quality AT services throughout<br />
the U.S. and are assisting seniors in living, learning, working and<br />
playing. This presentation will focus on putting knowledge into<br />
action, sharing models and exploring AT trends to watch.<br />

New Hampshire’s Alliance for Healthy Aging, a collective impact<br />
approach to address aging issues. The Alliance is a state wide<br />
collaboration of numerous, diverse stakeholders working toward<br />
a shared agenda and vision in creating an age friendly NH.<br />

Community-Based Falls Prevention <strong>Program</strong><br />

This session will provide an overview of the design and evaluation<br />
of an educational falls prevention program for communitydwelling<br />
older adults. The five-week program was developed<br />
based on sound theoretical underpinnings to reduce fear of<br />
falling and enhance falls efficacy and the program was evaluated<br />
using a pre- and post-test design for effectiveness and participant<br />
satisfaction.<br />

Innovative Implementation of Falls<br />

The CDC developed STEADI (Stopping Elderly Accidents, Deaths<br />
& Injuries) with the intent of providing tools and educational<br />
materials to identify patients at various levels of risk for a fall,<br />
identify modifiable risk factors, and offer effective interventions.<br />
