Table of Contents<br />
The table of contents on the opposite page is your guide to this<br />
program and an excellent reference to finding the information you<br />
need. Take a moment to browse through it to get an overview of<br />
the program, and refer back to it frequently as needed.<br />
<strong>Conference</strong> Hotels<br />
All conference activities will take place at the Hyatt Regency<br />
Chicago. Look for floor plans on pages 174–175.<br />
How to Use This Book<br />
This program book for the 2017 Aging in America <strong>Conference</strong> is designed to help you<br />
make the most of your experience at the conference. It has been organized to help you<br />
identify the activities that best serve your needs and interests, as well as plan your time<br />
at the conference more efficiently.<br />
Session Listings<br />
<strong>Conference</strong> session listings are organized by day, followed by<br />
time slot, session type, focus area and then session number. The<br />
daily session listings start on page 57. Under the title of each<br />
session you will find the time and location of that session. Each<br />
listing also provides a short description of the session content<br />
and presenters. To make it easy to find session descriptions, all<br />
sessions are numbered chronologically according to the day<br />
upon which they fall (Monday sessions begin with MO followed<br />
by the session number, Tuesday is TU, Wednesday is WE,<br />
Thursday is TH, and Friday is FR.).<br />
Finding What Interests You<br />
This book provides a number of ways to identify what interests<br />
you. You can identify specific sessions by subject area (see index,<br />
page 163), search by presenter (see index, page 153), or you can<br />
peruse the schedule starting on pages 4-5, which provides an<br />
overview of conference activities. Full descriptions of sessions<br />
begin on page 57 and are organized by date and time.<br />
Check the Addendum<br />
Changes to the program that occurred after this book went to<br />
print will be available electronically and shown continuously on<br />
a screen in the registration area. Be sure to check it often for<br />
updates to the schedule.<br />

TH245 Cantata’s Experience With a New Model<br />
of Home Support<br />
3:00 PM | Soldier Field (West Tower, Bronze Level)<br />
Learn about a new model of home care called Take2 that enables<br />
elders to get the help at home that they need, when they need it.<br />
Teams of six work in neighborhoods and visit up to 40 clients in<br />
an eight-hour shift. We will explain the model then discuss the<br />
benefits and potential drawbacks from three perspectives:<br />
provider/business, team members, and clients/family.<br />

Michelle Sherbun, PCC, CEO; both of Cantata Best Life Foundation.<br />

admitted to sessions without a badge. Exhibit-only badges are ineligible for access to educational<br />
programs. Some events require a ticket. Admission to those events requires both wearing a conference<br />
