the Home Care Registry<br />

A consumer who chooses to live at home needs a quality home<br />

care registry and, in many cases, assistance in hiring and<br />

sustaining the perfect caregiver. This session will provide insight<br />

into the development of a highly efficient quality home care<br />

registry and the role of consumer mentors and care coaches.<br />

Both consumer mentors and care coaches work with the<br />

consumer throughout the process to ensure a positive and<br />

sustainable relationship between the consumer and caregiver.<br />

MO171 Plan Your Lifespan: Developing<br />

and Testing a Tool for Planning<br />

in Advance for Major Life Events<br />

While many people plan for life events like buying a house or<br />

retirement, many fail to plan for health events that may occur<br />

in the last quarter of life, such as hospitalization, falls or<br />

memory loss. A tool for planning in advance for major life<br />

events developed with input from older adults and caregivers<br />

will be presented, as well as outcomes from a randomized<br />

control trial of the tool.<br />

MO173 Twenty Years and Counting: A Sustainable<br />

Model for Helping Seniors Age in Place<br />

Umbrella of the Capital District helps seniors remain in their<br />

homes and communities by recruiting retirees who wish to<br />

remain active and earn a little extra spending money to do home<br />

maintenance and provide other non-healthcare-related assistance.<br />

We will share what we have learned over our 20-year<br />

history and provide a model for creating a self-sustaining<br />

nonprofit to help seniors age in place.<br />

businesses at risk. Some of these risks are avoidable with ample<br />

planning and understanding within the workplace. Good risk<br />

management and prevention strategies can be used to generate<br />

improvements in your business. This workshop will help reframe<br />

the challenges of your current workplace.<br />

for Health Promotion <strong>Program</strong>s<br />

Health promotion programs inform and empower patients to<br />

make better healthcare choices, connect patients to services,<br />

improve patient activation and increase patient health confidence.<br />

However, those who work in health promotion often<br />

struggle to make the financial argument to maintain or expand<br />

their programming. This workshop will discuss how Rush Health<br />

& Aging established a business case for their health promotion<br />

programming.<br />

to combat ageism in the fashion industry. What can fashion<br />

schools and fashion houses do to change the culture? What<br />

Most baby boomers, like other Americans, want to die at home<br />

with loved ones. But one out of every five U.S. adults will die in<br />

the ICU. This session will educate participants on research<br />

conducted with patients, their loved ones, caregivers, providers<br />

and other experts in order to develop innovative tools that will<br />

help aging Americans understand treatment options and<br />

communicate their values and goals in a way that builds trust<br />

with their doctors and loved ones.<br />

Since 1996 the Family Caregivers Network has provided K-C<br />

employees information, support and advocacy on a variety of<br />

caregiving issues and awareness of essentials for a better life<br />

for elders or adults with disabilities. Learn about a geriatric<br />

care manager program created by the Network to better assess<br />

the value and requirements for meeting the growing demand<br />

for assistance.<br />
