caregivers. This session will detail challenges faced by family<br />
caregivers, strategies for addressing those challenges across<br />
poor, rural, diverse communities, and collaborative implementation<br />
of these strategies by business, nonprofits and state,<br />
local and tribal governments.<br />

WE403 Caregivers of Adults With Mental Illness:<br />
Results of a National Study<br />

This session will present the results of a national study conducted<br />
by the National Alliance of Caregiving in collaboration with<br />
the National Alliance on Mental Illness and Mental Heath<br />
America. This is the first national survey of caregivers of an<br />
adult with mental illness, and the presenters will discuss the<br />
challenges faced by these caregivers and suggest policy<br />
solutions to these issues.<br />

Without a Care Plan?<br />

Without essential knowledge, skills and proven processes,<br />
professionals are not able to use information gathered from<br />
caregiver assessments. This presentation will identify the types<br />
of tools, knowledge and skills needed to create viable and<br />
acceptable care plans to meet caregiver’ needs, and will discuss<br />
implications for selecting and implementing an array of evidence-based<br />
interventions.<br />

Making a Better <strong>Program</strong> for Patients<br />

Learn how Catholic Charities used lessons learned to design a<br />
more effective program for patients. A collaboration with Sinai<br />
Health Systems, the program targets patients at highest risk and<br />
provides ongoing case management to reduce readmission.<br />
Data shows that focused, continuous case management and<br />
coordination between the healthcare system and CBOs reduces<br />
costs overall and increases patient health and satisfaction.<br />

limited-English-speaking consumers to prevent and report<br />
healthcare fraud, the Illinois SMP at AgeOptions and the<br />
Coalition of Limited English Speaking Elderly led discussion<br />
groups with older adults in Cook County. We will discuss the<br />
process, lessons learned and possibilities for replication.<br />

Why Malnutrition Matters<br />

Learn about malnutrition’s impact on healthcare costs/outcomes<br />
and quality of life. Community-based organizations<br />
(CBOs) face an increased number of seniors with malnutrition;<br />
this emerging reality also offers opportunities for new partners/<br />
funding sources. A coalition of public/private organizations is<br />
launching a CBO “Malnutrition Toolkit” to link malnutrition care<br />
across the continuum and share important resources for older<br />
adults and caregivers to identify and address malnutrition.<br />

Healthcare and Aging Network (HAN).<br />
The national pilot projects to better serve people with both<br />
Medicare and Medicaid have fostered creative partnerships<br />
between aging and healthcare organizations. Using Alzheimer’s<br />
as an example, develop skills and acquire tools to approach<br />
