A parent/guardian must appear in person to the issuing office (LHS office for studentsresiding within the Lebanon School District) to give parental approval by signing theApplication for Employment Certificate. A birth certification record must also be presentedat this time.2. The student must then have the application completed by the prospective employer and aphysician.3. Upon receipt of the completed application in the LHS office, the work permit will be mailedto the employer.X. 11th GRADE PHYSICAL EXAM REQUIREMENTSAccording to Pennsylvania State Law, students in Grade 11 must have a physical examinationduring the year. It can be accomplished by:1. A scheduled exam at the school during school hours by the school doctor, free of charge.2. An examination by a school doctor for sports, free of charge.3. An examination by your family doctor, at your expense (obtain a school form from theMedical Department).Students who do not have an examination will not receive their 12th grade schedule.Y. TECHNOLOGY PROCEDURES (CELL PHONES, AUDIO/VIDEO PLAYERS, CAMERAS)Although convenient, cell phones are a distraction in the school setting. The distractions ofmodern technology include: text messaging of test/quiz answers, pictures of tests/quizzes witha camera phone, answering calls during class/lunch, sending or responding to text messages,playing video games or listening to music during the school day, and the theft of such items byother students.Cell phones, audio/video players, cameras, and other technology may not be used, seen, ordisplayed within the school building (including the cafeteria and courtyard). Violations will resultin disciplinary action and confiscation of the technological device.25
