7. <strong>You</strong> will get a letter in the mail telling you if your MEDI-CAL application isapproved or denied. If you have a MEDI-CAL health care plan, you will get ahealth care plan identification card in add<strong>it</strong>ion <strong>to</strong> the State-issued BIC.8. If you do not get an answer <strong>to</strong> your MEDI-CAL application w<strong>it</strong>hin a monthafter you apply, call your county eligibil<strong>it</strong>y worker.9. WHAT DO I NEED FOR VERIFICATION (PROOF)?<strong>You</strong> must give certain information before your MEDI-CAL can be approved.<strong>You</strong>r county eligibil<strong>it</strong>y worker will tell you what proof is needed.<strong>You</strong> may apply w<strong>it</strong>hout the proof, but you will have <strong>to</strong> give <strong>it</strong> later. If youcannot get the proof yourself, ask your county eligibil<strong>it</strong>y worker <strong>to</strong> help you.ITEMS REQUIRED for full MEDI-CAL benef<strong>it</strong>s (if applicable):1. Social Secur<strong>it</strong>y card(s).2. <strong>Medi</strong>care card(s).3. Proof of U.S. c<strong>it</strong>izenship or U.S. national status.4. Alien registration card(s) or other immigration documents and ident<strong>it</strong>y(some groups are exempt).5. Pregnancy verification.6. Income verification:a. Employee pay stubs or a statement from your employer showing grossearnings and deductions.b. Award letter or checks showing amount of pension or benef<strong>it</strong>s, includingSocial Secur<strong>it</strong>y and V.A.c. State Unemployment or Disabil<strong>it</strong>y award letter.d. Student Loan grant award letter(s) or loan grant papers.e. Statement from providers of other income (contributions, refunds, childsupport, etc)f. Self-employment information: Last year’s tax return or current ledgers,current inven<strong>to</strong>ry, including business equipment and supplies.g.
