התמודדות עם סיבוכי המחלהגישות חדישות בסרטן הערמוניתפרופ' אבישי סלעמנהל המכון האונקולוגי,המרכז הרפואי אסף הרופא, צריפיןבמאמר זה אתמקד בגישות החדישות לסרטןערמונית בשלב העמידות לטיפול לדיכויהורמון המין הגברי. תוחלת החיים של החוליםעם טיפול מבוסס טקסוטר היא כ-<strong>18</strong>.5חודשים, כאשר לאחר התקדמות המחלהבטיפול מבוסס טקסוטר תוחלת החיים היאכ-12 חודשים. אתעכב על מספר מנגנוניםואתמקד רק בתרופות במחקרי פאזה IIIהקשורות במנגנונים אלה.יצירת כלי דם — Angiogenesisאנגיוגנזיס הוא אחד המנגנונים התורמיםלשונות המורכבת של סרטן הערמונית. מידתהאנגיוגנזיס הנמדדת בצפיפות כלי הדם מלווהבהתמיינות גבוהה של הגידול, בכישלון ביוכימילאחר כריתת הגידול ובתוחלת חיים נמוכה.גידולים אגרסיביים יוצרים כלי דם בעל מבנהותפקוד פרימיטיביים, ומלווים בגרורות ]1[בהקשר הזה מתקיימים מספר מחקריםקליניים פאזה :IIIמחקר CALGB9040, המיועד לחוליםעם מחלה עמידה לדיכוי הורמון המיןהגברי ומשווה את הטיפול המקובלטקסוטר/פרדניזון בשילוב תרופת המחקרבווסיזומאב )Bevacizumab( לעומתתשלובת הטקסוטר ופלצבו. מחקר זה גייס1,020 חולים ולפי השמועות המידע יוצגבכינוס ASCO הקרוב.מחקר ה-Venice, המשווה את תשלובתטקסוטר ופרדניזון בשילוב עם VEGF-tra-ablivercept לעומת תשלובת טקסוטר/פרדניזון ופלצבו. מחקר זה, המתנהל כעת,סיים לאחרונה גיוס 1,200 חולים. תרופתהמחקר הינה חלבון דמוי קולטן ל-VEGF)Vascular Endothelial Growth Factor(עם התקשרות גבוהה ל-VEGF וטווחפעילות רחב ]2[.מולקולה נוספת בעלת חשיבות היאסוטנט .)Sutent( בחולים לאחר טיפולבטקסוטר שיעור התגובה הביוכימיתresponse( )PSA נמצא 9% ו-11.1%בשני מחקרים. שיעור התגובה המדידה:תגובה חלקית — ,11.1% יציבות — ,44.4%משך השליטה במחלה — 21.1 שבועות;ב-78.9% מהמטופלים נשלטה המחלהבמשך 12 שבועות ]3,4[. בשל רעילותהטיפול, שונה המינון המקובל של 50מ"ג ליממה על בסיס של 4/2 שבועותל-37.5 מ"ג ליממה, באופן מתמשך,במחזורים בני ארבעה שבועות. מחקרפאזה ,III המתנהל כעת ומיועד לחוליםלאחר כישלון טיפול בטקסוטר, יכלול 819חולים ויעדו הוא הארכת תוחלת החייםמ-12 חודשים ל-16.2 חודשים — הארכהשל — 35% באמצעות תשלובת סוטנט/פרדניזון לעומת פלצבו/פרדניזון.התמקדות בציר האנדרוגניבהעדר אנדרוגן, הקולטן לאנדרוגן אינו פעילוקשור למערכת .HSP90 Chaperone נוכחותאנדרוגן תגרום לשינויים מבניים בקולטן,לדה-מריזציה, להתקשרות למרכיבים תלויי-אנדרוגנים ולשכתוב גנים המהווים מטרהלקולטן האנדרוגני.הקולטן לאנדרוגן פעיל או עובר שינוי בנוכחותרמות נמוכות של הורמון המין הגברי, כאשרסרטן הערמונית העמיד לטיפול הורמונלי עדייןתלוי בהתפתחותו בקולטן לאנדרוגנים. כך,אנו מוצאים בגידול רמות גבוהות של הורמוןהמין הגברי ותוצריו בסרטן ערמונית עמידלדיכוי הורמון המין הגברי. לכן, מהווה הקולטןלאנדרוגנים איבר מטרה לטיפולים חדישים ]5[.במערך היווצרות הורמון המין הגברי ותוצריומהווה האנזים CYP17 מרכיב עיקרי, כאשררמות מוגברות שלו נמצאות בסרטן ערמוניתעמיד לרמות נמוכות של הורמון המין הגברי.בהקשר הזה, אבירטרון )Abiraiterone( מעכבאת יצירת הורמון המין הגברי באשך, ביותרתהכליה ובערמונית, ופעיל יותר מקטוקונזול.בחולים עם סרטן ערמונית עמיד לדיכוי הורמוןהמין הגברי נצפתה ירידת PSA של 50%במחצית המטופלים, וירידת PSA של 90%נצפתה ב-20%-30% מהמטופלים. ירידה זושל PSA לוותה בתגובה מדידה בגידול, בירידהבספירת תאי גידול בדם ובשיפור קליני ]6[.תגובה זו הייתה גבוהה יותר לפני מתן טיפולכימי מאשר לאחר טקסוטר, וירדה לאחרטיפול בקטוקונזול ]7,8[. מחקר גדול בחוליםלאחר טיפול כימי בטקסוטר המשווה טיפולבאביטרון/פרדניזון לעומת פלצבו/פרדניזוןסיים לאחרונה לגייס 1,158 חולים. יעדהמחקר הוא הארכת תוחלת החיים.תרופה נוספת היא .MDV3100 לתרופה זומנגנון חסימה ייחודי של הקולטן לאנדרוגןברמת הטרנסלוקציה הגרעינית של הקולטן.התרופה פעילה בתאים עם ביטוי יתר שלהקולטן, העמידים לטיפול ב-Casodex )מעכבאחר של הקולטן לאנדרוגן( ]9[.97
