nuchal translucency. Down syndrome (DS) screening can be performed
with 95% sensitivity with the addition of vascular measurements.
Recommended for all pregnant women
Diagnostic tests for Down syndrome and other chromosome diseases
require fluid or tissue to be taken from the uterus or placenta with needle
entry. For this reason, the risks that may arise are shared with the family
by the doctor and these tests, which require experience, are important in
terms of risks in safe centres and expert hands. Screening tests are not
mandatory but are recommended for all pregnant women. Every situation
that appears to be risky as a result of tests does not require termination
of pregnancy. However, the treatment of the postnatal baby should be
started immediately.
Healthy pregnancy does not eliminate the risk of disease
The social environment may adversely affect the family’s preference for
conducting tests. For example, the expectant mother may have of the
opinion that she / he doesn’t use cigarettes or alcohol because she / he
is paying attention to diet and exercise, and that such a situation will not
occur in her own baby. And she may also have the opinion that she is not
going to have a baby with Down syndrome since there is no other family
member with Down syndrome or with other genetic diseases as well.
However, many of the individuals with Down’s syndrome have not had
such a condition in the family, and the expectation of a healthy pregnancy
will not change this risk.
Advanced maternal age increases the risks in pregnancy
Down syndrome is a disorder that can be defined during pregnancy or at
birth. It was found at 1 of 700 births and statistics show that this rate
is being increased with the increase in maternal age. However, it may
also occur in pregnancies younger than 35 years. Screening tests for DS
indicate whether the baby is at high or low risk for this syndrome. These
screening tests, in addition to chromosomal abnormalities, also identify
risks for sex chromosome abnormalities, which are usually associated
with mild mental retardation and affect fertility.
Termination of pregnancy depends on the wish of the family
Some tests recommended to be done during pregnancy may be rejected
by families. This is because the family do not want to accept the
termination of pregnancy if the baby is going to be born with a genetic
kazanmaktadır. Tarama testlerinin
yapılması zorunlu olmasa da tüm gebelere
önerilir. Testler sonucu riskli görülen
her durum, gebeliğin sonlandırılmasını
gerektirmez. Ancak doğum sonrası bebeğin
tedavisine vakit kaybetmeden başlanması
gerekir.
Sağlıklı gebelik geçirmek, hastalık
riskini ortadan kaldırmaz
Sosyal çevre, ailenin testleri yaptırma
konusundaki tercihini olumsuz yönde
etkileyebilir. Örneğin anne adayı, diyet
ve egzersizine dikkat ettiği için, sigara
veya alkol kullanmadığından ve ailesinde
başka down sendromlu ya da diğer
genetik hastalıklı bir birey olmadığı için
kendi bebeğinde de böyle bir durumun
oluşmayacağı kanısına varabilir. Fakat
down sendromu olan bireylerin çoğunun
ailesinde daha önceden böyle bir
durum görülmemiştir ve anne adayının
sağlıklı bir gebelik geçirilmesi, bu riski
değiştirmeyecektir.
Gebelikte ileri yaş riski artırıyor
Down sendromu gebelik sırasında ya da
doğumda tanımlanabilen bir rahatsızlıktır.
700 doğumda 1 oranında rastlanır ve
istatistikler, anne yaşının artışıyla bu
oranın yükseldiğini göstermiştir. Ancak
daha genç yaş grubu olan 35 yaş altındaki
gebeliklerde de ortaya çıkabilir. DS
için olan tarama testleri, bebeğin bu
sendrom için yüksek veya düşük riskli olup
olmadığını gösterir. Yapılan bu tarama
testleri, kromozom anormalliklerinin
yanı sıra genellikle hafif mental gerilikle
seyreden ve doğurganlığı etkileyen cinsiyet
kromozomu anormallikleri için de riskleri
tespit eder.
Gebeliğin sonlandırılması ailenin
isteğine bağlıdır
Gebelik sürecinde yaptırması önerilen bazı
testler, aileler tarafından reddedilebilir.
Bunun nedeni, bebeğin genetik bir hastalık
ile dünyaya gelme durumu söz konusuysa,
gebeliğin sonlandırılmasını kabul etmek
istememelerinden kaynaklanmaktadır.
Testlerin sonucuna göre bebekte
doğumsal bir anomali ya da genetik bir
sorun söz konusuysa, gebelik yalnızca
ailenin onayı ile sonlandırılabilir.
disease. If the baby has a congenital anomaly or a genetic problem, the
pregnancy can only be terminated with the consent of the family.
