• $25,000 in the event of death due to any cause for an employee who is 65 to 69 years of age<br />

• benefit is paid regardless of cause of death based on employee's eligibility at date of death<br />

Continuation of Coverage<br />

• the Employer will continue to pay the Group Life contributions while the employee is receiving<br />

sick pay, is on maternity or parental leave, or during the first 20 work shifts in any calendar year<br />

of unpaid leave<br />

• coverage can continue while an employee is on an unpaid leave, if the employee pays 100% of<br />

the contributions<br />

• while an employee receives LTD benefits from the Plan, the employee's Group Life coverage will<br />

continue at no cost to the employee or the Employer as long as the employee remains an<br />

employee<br />

Termination of Coverage<br />

Coverage ceases on the date the employee:<br />

• terminates employment<br />

• retires<br />

• commences an unpaid leave beyond 20 work shifts in any calendar year and employee elects not<br />

to pay contributions or elects to pay contributions and then stops paying them until their return to<br />

work<br />

• transfers to an ineligible status<br />

• is laid off<br />

• turns 70 years of age<br />

Conversion<br />

• upon termination of employment (excluding retirement), coverage continues at no charge to the<br />

employee or Employer for 31 days during which time the employee may convert all or part of<br />

their group life insurance, without providing medical evidence, into any whole life, endowment or<br />

term life policy normally issued by the insurer at the insurer's standard rates at that time<br />

Advance Payment Program<br />

• in the event of terminal illness, with medical information confirming life expectancy of less than<br />

one year, an advance payment of up to 50% of the Group Life benefit, subject to a maximum of<br />

$25,000, is available to the employee<br />

ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)<br />

Premiums<br />

• 100% employer-paid<br />

Eligibility<br />

• regular full-time and regular part-time employees scheduled to work 20 regular hours or more per<br />

week except as provided in MOA #6 (Re: Health and Welfare Benefits Entitlement Threshold)<br />

• enrolment is a mandatory condition of employment
