• the final report is signed by all members of the Committee and forwarded in writing to the carrier<br />
who is then responsible for forwarding a copy to the employee, Employer and the Union<br />
• expenses of the Chairperson are shared equally between the employee (or Union) and the carrier;<br />
expenses of the two nominees are the responsibility of each appointing party; expenses for<br />
medical procedures requested by the Committee, and travel expenses of the employee are the<br />
responsibility of the employee (or Union)<br />
DENTAL<br />
Premiums<br />
• 100% employer-paid<br />
Eligibility<br />
• regular full-time and regular part-time employees scheduled to work 20 regular hours or more per<br />
week except as provided by MOA #6 (Re: Health and Welfare Benefits Entitlement Threshold)<br />
• enrolment is a mandatory condition of employment<br />
Dual Coverage Restriction<br />
• employees and/or dependants are ineligible for coverage if enrolled in another dental plan that is<br />
equal or better to this dental plan<br />
Dependants<br />
• husband, wife, common-law spouse (spousal partners who have co-habited for a period of not less<br />
than one year)<br />
• unmarried children until the end of the month in which they turn the age of 19 years, if they are<br />
mainly dependent on and living with the employee or the employee's spouse<br />
• unmarried children until the end of the month in which they turn the age of 25 years, if they are in<br />
full-time attendance at a recognized school, college or university, if mainly dependent on the<br />
employee or the employee's spouse<br />
• unmarried physically or mentally handicapped children to any age, if mainly dependent on and<br />
living with the employee or the employee's spouse<br />
Effective Date<br />
• first day of the month following the month in which the employee successfully completes their<br />
probation or trial period not to exceed three months<br />
• orthodontic coverage for the employee and dependants takes effect 12 months after enrolment of<br />
the employee in the dental benefit<br />
Basic <strong>Services</strong><br />
100% reimbursement for:<br />
• diagnostic services:<br />
o one standard exam every nine months for adults or twice in any calendar year for children<br />
under 19 years of age<br />
o one complete exam in any three year period, provided no other exam has been paid by the<br />
Plan in the preceding nine months for adults or preceding six months for children under<br />
19 years of age<br />
o x-rays, up to the maximum established by the carrier for the calendar year
