Effective Date<br />
• first day of the month following the month in which the employee successfully completes their<br />
probation or trial period not to exceed three months<br />
Amount of Benefit<br />
• $50,000 (principal sum) in the event of death due to any accidental cause (in addition to the<br />
Group Life benefit) for an employee who is less than 65 years of age<br />
• $25,000 (principal sum) in the event of death due to any accidental cause (in addition to the<br />
Group Life benefit) for an employee who is 65 to 69 years of age<br />
• 100% of principal sum in the event of loss of both hands, or both feet, or sight of both eyes, or<br />
one hand and one foot, or one hand and the sight of one eye, or one foot and the sight of one eye,<br />
or hearing in both ears and speech<br />
• 75% of principal sum in the event of loss of one arm or one leg<br />
• 50% of principal sum in the event of loss of one hand, or one foot, or sight of one eye, or hearing<br />
in both ears, or speech<br />
• 25% of principal sum in the event of loss of thumb and index finger of one hand, or all four<br />
fingers of one hand<br />
• 12.5% of principal sum in the event of loss of all toes of one foot<br />
Exclusions<br />
• suicide or attempted suicide, while sane or insane<br />
• intentionally self-inflicted injury<br />
• war, insurrection or hostilities of any kind, whether a participant or not in such actions<br />
• participation in any riot or civil commotion<br />
• bodily or mental infirmity or illness or disease of any kind, or medical or surgical treatment<br />
thereof<br />
• travel or flight in any aircraft except solely as a passenger in a powered civil aircraft having a<br />
valid and current airworthiness certificate, and operated by a duly licensed or certified pilot while<br />
such aircraft is being used for the sole purpose of transportation only - descent from any aircraft<br />
in flight will be deemed to be part of such flight<br />
• committing or attempting to commit a criminal offence or provoking an assault<br />
• in the course of operating a motor vehicle while under the influence of any intoxicant; or, if blood<br />
alcohol concentration is in excess of 100 milligrams of alcohol per 100 millilitres of blood<br />
Continuation of Coverage<br />
• the Employer will continue to pay the AD&D contributions while the employee is receiving sick<br />
pay, is on maternity or parental leave, or during the first 20 work shifts in any calendar year of<br />
unpaid leave<br />
• coverage can continue while an employee is on an unpaid leave, if the employee pays 100% of<br />
the contributions<br />
• while an employee receives LTD benefits from the Plan, the employee's AD&D coverage will<br />
continue at no cost to the employee or the Employer as long as the employee remains an<br />
employee
