Primary Care Trusts (PCTs) have played the critical<br />

role of commission<strong>in</strong>g primary and secondary services<br />

for providers <strong>in</strong> the NHS<br />

• PCTs have set their own budgets and priorities with<strong>in</strong> the overarch<strong>in</strong>g budgets and priorities of the SHAs.<br />

• Prior to the reforms, a total of 152 PCTs existed throughout England, each oversee<strong>in</strong>g an average<br />

population of 330,000.<br />

• PCTs have thus far controlled 80% of the <strong>healthcare</strong> budget.<br />

• The Department of Health allocates revenue for NHS services to its PCTs based on:<br />

– A weighted capitation formula – tak<strong>in</strong>g <strong>in</strong>to account population characteristics (ie: size, age, deprivation levels) and<br />

unavoidable market forces account<strong>in</strong>g of variations <strong>in</strong> costs.<br />

– Recurrent allocations – previous f<strong>in</strong>ancial year’s allocations adjusted, for example, for any newly devolved central budgets<br />

and transfer of responsibilities and their associated budgets between PCTs.<br />

• Its total revenue allocations <strong>in</strong> 2011-12 were £89 billion.<br />

• The revenue allocations for 2012-2013 were announced to be £91.6 billion, an <strong>in</strong>crease of £2.5 billion, or<br />

2.8%.<br />

– The weighted capitation formula was not applied for 2012-2013 revenue allocations as all PCTs received a uniform uplift so as to ma<strong>in</strong>ta<strong>in</strong> f<strong>in</strong>ancial<br />

stability with<strong>in</strong> the NHS at a time of signifi<strong>can</strong>t transition.<br />

• This is the last round of allocations made to PCTs as from 2013 onwards, the NHS Commission<strong>in</strong>g Board<br />

would be responsible for the commission<strong>in</strong>g of resources to Cl<strong>in</strong>ical Commission<strong>in</strong>g Groups (CCGs) while<br />

the Department of Health will make grants to Local Authorities for public health needs.<br />

• The Health and Social Care Act 2012, is call<strong>in</strong>g for all PCTs to be abolished by April 1, 2013.<br />
