Public–Private Interaction in Health: Policy IssuesSession 3Public–Private Interaction inHealth: Policy IssuesRamesh Bhat, Professor, Indian Institute of Management, AhmedabadIntroductionThe role of the private sector in provision and financing assumes considerableimportance in recent times. Recent health financing patterns suggest that the outof-pocketcost on health accounts for about 78% of the total expenditure onhealth (World Bank, 1997). Private health expenditure is estimated to be about4.25% of the gross domestic product (GDP). Insurance coverage mechanisms arenegligible, and most of that expenditure is out-of-pocket (i.e., private financing).Private health care expenditure in India has grown at the rate of 12.5% perannum. For each 1% increase in per capita income, private health careexpenditure has increased by about 1.47% (Bhat, 1996).The share of the private sector in health infrastructure is also quite significant.About 57% of hospitals and 32% of hospital beds are in the private sector. Theshare of private sector investment in health infrastructure (e.g., hospitals,investment in medical equipment and technology, etc.) is also quite significant.Most of the qualified doctors work in the private sector; data suggests that atpresent about 80% of 390,000 qualified allopathic doctors registered withmedical councils in India and 650,000 providers from other branches ofmedicine are working in the private sector. Dependence on the private sector is,therefore, considerable. Utilization studies also show that one-third of inpatientsand three-fourths of out-patients use private health care facilities(Duggal and Amin, 1989; Yesudian, 1990; Visaria and Gumber, 1994).Given the presence of the private sector, various state governments and centresupportedprogramme strategies in the health sector emphasize public–privatecollaborations. The role of private initiatives is considered important in view ofgrowing concerns about shrinking budgetary support from the government tocurative and tertiary health care, increasing burden arising from dual burden(communicable and non-communicable) of diseases, and the growingcomplexities in the areas of technology, patient–provider relationships, anddelivery mechanisms of health care services.99
