Standard Three 13• Reasons for enrolling and/or a program of study• Identification of a disability• For students not enrolled the previous threequarters, it also asks:• Race• Previous schools attended• How long they plan to attend <strong>Centralia</strong> <strong>College</strong>• Work status• Prior education level• Family statusAnalysisThe registration form is designed at the state consortium level, and theinformation from each student is entered in the computerized studentmanagement system (SMS). This information is used for enrollmentmanagement and institutional research purposes (retention patterns,enrollment trends over time, comparisons with other colleges in thesystem), to provide statistical data for grant applications and for fundingpurposes. (See Attachment 3.4)The "reasons for enrolling" information is primarily used for reporting tothe State Board for retention analysis, and for tracking numbers ofstudents in professional/technical (vocational) programs for federalfunding. (See Attachment 3.5, 3.6, 3.7, 3.8) It is useful for programplanning to identify the demand level for a program. Also, students couldbe contacted by a particular professional-technical program advisor or besent transfer material.The disability information is reported to the State Board and is factoredinto determining allocations of accommodation grant funds to the <strong>College</strong>.Though all new students receive information about the availability ofaccommodations, names of those who self-identify are not sent to thespecial needs director for outreach. Some students do not want to beidentified and choose not to receive services.Much of the other information asked on the back of the registration formappears to be redundant, as it is already collected on the admissions
