Annex 34Record of Reports ReceivedDistrict surveillance personnel should use this <strong>for</strong>m to record the timeliness and completeness of themonthly reporting that it receives from health facilities. The <strong>for</strong>m allows the surveillance personnel to monitorthe reporting per<strong>for</strong>mance of each health facility in the district so that action can be taken to improve unsatisfactoryper<strong>for</strong>mance. The <strong>for</strong>m can be adapted <strong>for</strong> monitoring weekly and quarterly reporting.Please note that timeliness and completeness are expressed as percents (%).
