(See Section 3.)Health staff record the totals on a recommended monthly summary reporting <strong>for</strong>m.Note: In the sample monthly summary reporting <strong>for</strong>ms, there is space <strong>for</strong> recording observations about thedata that health professionals at the health facility and district observe either during routine analysis or whenthey complete the <strong>for</strong>m each month.2.4.2 Submit zero reporting when no cases of immediately reportable diseases are diagnosedIf no case of a particular disease is detected in the reporting period, put a “zero” (0) <strong>for</strong> that disease on the<strong>for</strong>m. This will tell the staff at the next level that the health facility or district has submitted a complete report,i.e., that another number was not inadvertently omitted.2.4.3 Use line lists and summary reporting during outbreaksWhen five or fewer cases of a single disease occur during the week in a health facility, report the in<strong>for</strong>mationabout each case on an individual case-based reporting <strong>for</strong>m.If more than five cases occur during a week per health facility, use a line list instead of an individual casebasedreporting <strong>for</strong> to record and report the cases weekly.When a large number of cases occur in a single suspected outbreak, report summary totals of cases anddeaths each week.Section 2: Report Priority <strong>Disease</strong>s and Conditions 27
