The procedure must be performed as a direct result of the carcinoma in situ andspecifically to arrest the spread of malignancy, and be considered the necessary andappropriate treatment• skin cancer other than melanoma at least 1.5 mm thick or at least Clark Level 3 of invasion• hyperkeratosis or basal cell skin carcinoma• squamous cell skin carcinoma unless it has spread to other organs.Severe infection in the blood stream that causes blood poisoning which results in a permanentimpairment of at least 25% of whole person function.The progressive weakening and wasting of the muscles of the body. The diagnosis of motorneurone disease must be certain and supported by neurological investigations. There must bepermanent loss of at least 25% of the brain’s physical control function.The progressive destruction of the insulating layer of myelin in the brain and spinal cord. Thediagnosis of multiple sclerosis must be certain and supported by neurological investigations.There must be more than one episode of brain dysfunction with persistent abnormalities and lossof at least 25% of either the brain’s mental function or its physical control function.The progressive weakening and associated loss of muscle tissue resulting in a permanent loss ofat least 25% of whole person function.Open heart surgery for the treatment of a cardiac defect, cardiac aneurysm or benigncardiac tumour.Cardiac arrest which is not associated with any medical procedure and is documented by anelectrocardiogram, occurs out of hospital and is due to cardiac asystole or ventricular fibrillationwith or without ventricular tachycardia.Cardiac arrest as a direct or indirect result of alcohol or drug intake is excluded.A degenerative disease of the central nervous system, the most common symptom of which is arhythmic tremor of the limbs. The diagnosis of Parkinson’s disease must be certain and supportedby neurological investigations.
