Setting upyour insuranceHow to applyHow do you apply for insurance?Your financial adviser can prepare a recommendationbased on your financial needs.Read this PDS, then apply for LCS and PPP using thecurrent application (the Trustee will use the applicationto apply for an LCS insurance policy on your behalf)and send it with your payment details to <strong>MLC</strong>. For moreefficient processing <strong>MLC</strong> prefers this payment by directdebit request or credit card authority.A LCS policy can only cover one life insured. Theinsurances form a part of a policy issued by <strong>MLC</strong>Limited to <strong>MLC</strong> Nominees Pty Limited as the Trustee ofthe Scheme.A PPP policy can cover up to six lives and each life insuredcan have different types of insurance and levels of cover.How is your application assessed?After receiving your application <strong>MLC</strong> assesses it,taking into account your occupation, lifestyle, leisureactivities and medical history, to decide whether, andon what terms or conditions, insurance can be offeredto you. In some cases you may not be eligible for theinsurance you request or <strong>MLC</strong> may offer you insuranceat a higher premium or with an exclusion (for instance,if you have an existing health problem or participate in ahazardous sport).<strong>MLC</strong> may telephone you to help assess and processyour application.To process your application <strong>MLC</strong> may require you toprovide additional medical information. You may berequested to undergo a medical examination or other test.If this is required, you will be notified and the appropriateforms will be sent to you. <strong>MLC</strong> will pay for medicalinformation requested during the assessment process,unless you are advised otherwise. <strong>MLC</strong> may also need towrite to your doctor for additional information (authorisedby you).You must let <strong>MLC</strong> know if any information you give onyour application changes before <strong>MLC</strong> has issued a policy(for instance, if you become ill or have an accident). Ifyou do not, the insurance policy may not be valid. Thisalso applies to applications to extend, vary or reinstatethe policy.Interim Accident InsuranceWhen your application is submitted to <strong>MLC</strong>, you will becovered by the Interim Accident Insurance Certificate onterms as detailed on pages 83 to 85.How do you appoint yourauthorised representative?You may appoint an authorised representative to accessinformation about the policy on your behalf with a writtenrequest, or by completing the Change of Policy DetailsForm. This person is not permitted to perform any otheractions in respect of the policy. The appointment isvalid until changed or cancelled in writing by you (in aform acceptable to <strong>MLC</strong>), or until the policy is cancelled.It is your responsibility to change your authorisedrepresentative if your circumstances change.What payment will yourfinancial adviser receive?Your premium includes provision for your financialadviser’s commission, but the actual remuneration yourfinancial adviser receives depends upon your financialarrangement with them, which will be specified in thestatement of advice they must provide to you. Thecommission is not an additional cost to you. The premiumwill not be less, if you don’t have a financial adviser.Need help?
