The diagnosis must be certain and supported by neurologicalinvestigations.The diagnosis of muscular dystrophy which is the progressive weakening and associated loss ofmuscle tissue. The diagnosis must be certain and supported by muscle biopsy or neurologicalinvestigations.The diagnosis of Parkinson’s disease which is a degenerative disease of the central nervoussystem. The diagnosis must be certain and supported by neurological investigations.The undergoing of skin grafting and plastic or reconstructive surgery above the neck which isdeemed medically necessary for the treatment of facial disfigurement as a direct result of an accidentrequiring inpatient hospital treatment of the life insured. An accident or injury as a direct or indirectresult of alcohol or drug intake is excluded. The accident must occur whilst the policy is in force.The life insured’s child, as a result of injury or sickness, will be permanently unable to do any two ormore of the following groups of activities of daily living without physical help from someone else:• bathing or showering• dressing• moving from place to place, in and out of bed and in and out of a chair• eating and drinking• using the toilet.The life insured’s child also suffers an inability to gain independence which results in permanentloss of at least 25% of either the brain’s mental function or its physical control function which leadsto a need for continuous supervision of the child to protect them or other people.A claim can only be made once the initial assessment or diagnosis is reconfirmed after six months.Benefits are not payable:if the inability of an existing or future child to gain independence is caused or contributed to by aninjury that occurred or sickness that first appeared before this Extra Benefits Option commencedor was last reinstated, andif payment has been made under ‘Congenital abnormalities of a child’, or ‘Death of a child’.The total and irrecoverable loss, or loss of the use, of the following:• one foot• one hand.Loss as a direct or indirect result of alcohol or drug intake is excluded.The permanent loss of sight in one eye, whether aided or unaided, due to sickness or injury to theextent that visual acuity is 6/60 or less.The presence of one or more malignant melanomas. The melanoma must be between 1.0 mm and1.5 mm Breslow thickness and also less than Clark Level 3 depth of invasion as determined byhistological examination. The malignancy must be characterised by the uncontrollable growth ofmalignant cells.The life insured has been placed on an Australian waiting list, approved by <strong>MLC</strong>, for an organtransplant (as listed under the Major Organ Transplant definition) from a human donor.The presence of prostate cancer histologically described as TNM classification T1(or another equivalent classification) and with a Gleeson score of at least six.Definitions<strong>MLC</strong> <strong>Life</strong> <strong>Cover</strong> <strong>Super</strong> and <strong>MLC</strong> <strong>Personal</strong> <strong>Protection</strong> <strong>Portfolio</strong> PDS Page 81
