1102.3: Forms of ApplicationThe Application for a Permit and a Certificate of Use and Occupancy shall be submitted insuch form as the <strong>Zoning</strong> Officer may prescribe and shall be accompanied by the requiredfee as prescribed in Section 1104.1102.4: Plot DiagramApplications shall be accompanied by a plot plan diagram in duplicate. The plot plan shallbe drawn to a suitable scale and shall clearly and accurately show dimensions of buildingsand lots, both existing and proposed, abutting streets, proposed uses of buildings and lots,north arrow and scale, and other information which the <strong>Zoning</strong> Officer may require tomake a decision on the permit. One copy shall be returned to the applicant indicatingapproval or disapproval, one copy shall be retained by the <strong>Zoning</strong> Officer.1102.5: Amendments to a PermitAmendments to a permit or other records accompanying it may be filed at anytime beforecompletion of the work. The <strong>Zoning</strong> Officer shall approve all such amendments except forthose to permit which have been authorized by action of the <strong>Zoning</strong> Hearing Board inwhich case the Board shall approve any amendments. Amendments shall be deemed partof the original application.1102.6: Expiration of PermitsIf work described in any permit has not begun within 90 days from the date of issuance, thepermit shall expire. A written notice shall be given by the <strong>Zoning</strong> Officer to the personsaffected. If permitted work has not been substantially completed within one year of thedate of issuance, the permit shall expire. A written notice shall be given by the <strong>Zoning</strong>Officer to the persons affected.Upon expiration of a permit, work may not continue until either a permit renewal or a newpermit has been obtained. The <strong>Zoning</strong> Officer may issue a permit renewal when theproposed activities, information, and conditions contained in the original permit applicationwill be continued without change. The permit renewal shall include limitations on time notto exceed one year allowed for substantial completion of the work. The <strong>Zoning</strong> Officermay require the provision of a reasonable performance bond to ensure completion with thetime limit specified in the permit renewal.74
