DOSSIERNotre travail permettra de préciser l’intérêt d’une implantation bilatéra<strong>le</strong>en la comparant à l’appareillage bimodal. Nous attacheronsune grande importance à l’amélioration des performances auditivesdans <strong>le</strong>s situations suivantes : l’écoute de la musique, la discriminationdans <strong>le</strong> bruit et la localisation spatia<strong>le</strong> ainsi qu’à la sensationsubjective d’amélioration de confort par <strong>le</strong> biais d’un questionnaire.2.3 ProblématiqueNotre problématique consiste d’abord à constituer deux groupes depatients comparab<strong>le</strong>s.Pour ce faire, nous avons col<strong>le</strong>cté <strong>le</strong>s données recueillies par l’IFICentre 2006 et 2011. Cel<strong>le</strong>s-ci se présentent comme une base dedonnées (midd<strong>le</strong>c@re) qui recense l’intégralité des caractéristiquesde 1600 patients et de <strong>le</strong>ur suivi. On trouve notamment, l’âge, <strong>le</strong>sexe, l’étiologie, des données qualitatives, des comptes rendusmédicaux, etc.Nous avons trié <strong>le</strong>s données de la base midd<strong>le</strong>c@re et retenu <strong>le</strong>spatients qui correspondent à nos critères d’inclusions. Nous avonstiré aléatoirement 9 patients parmi cette liste qui ont une implantationbilatéra<strong>le</strong> et 8 patients qui ont une implantation unilatéra<strong>le</strong> etun appareillage auditif controlatéral. Nous disposons ainsi de deuxgroupes comparab<strong>le</strong>s.S’il s’avère, qu’en moyenne, <strong>le</strong>s patients de l’un des deux groupesbénéficient de meil<strong>le</strong>ures performances auditives, au sens large,ainsi que d’un meil<strong>le</strong>ur confort qualitatif que l’autre groupe, alorsnous pourrons conclure à la supériorité d’une stratégie.Notre étude évalue <strong>le</strong>s bienfaits d’une stratégie en termes multifactoriels.En effet, l’amélioration attendue par une stratégie thérapeutiqueest toujours pluridimensionnel<strong>le</strong>. Nous restreindrons l’étude del’amélioration de la vie des patients aux facteurs suivants (écoute ensituation calme, en situation bruyante, écoute en situation d’écho,écoute de la musique, de la télévision, localisation spatia<strong>le</strong>, améliorationdes performances orthophoniques, amélioration des performancesaudiométriques).Nous ne testons qu’une partie des bienfaits éventuels des stratégiesthérapeutiques, notamment cel<strong>le</strong>s qui font l’objet de mesuresaudiométriques 2 .3 Méthode3.1 Critères d’inclusionNotre étude porte sur la comparaison des résultats d’interventionsur des patients qui satisfont aux critères d’inclusion suivant :- adultes entre 18 et 69 ans,- capacité cognitive norma<strong>le</strong>.- personnes francophones,- surdité bilatéra<strong>le</strong>,- surdité sévère à profonde,- surdité congénita<strong>le</strong> et/ou génétique,- arrivée de la surdité post lingua<strong>le</strong>,- implanté bilatéral ou appareillage bimodal (implant + aide auditive).Un patient est inclus dans l’échantillon s’il satisfait à tous <strong>le</strong>s critères.2. Nous ne mesurons pas, par exemp<strong>le</strong>, l’amélioration de l’insertion socia<strong>le</strong> despatients.3.2 Constitution des groupesLes patients qui composent <strong>le</strong>s deux groupes sont suivis par l’Institutfrancilien d’implantation cochléaire et ont été implantés dansplusieurs services d’implantation français répartis en Î<strong>le</strong>-de-Franceentre 1997 et 2010.Sur l’ensemb<strong>le</strong> des données de la base midd<strong>le</strong>c@re nous avonssé<strong>le</strong>ctionné <strong>le</strong>s patients qui correspondent à nos 8 critères d’inclusions.Ce premier tri a permis d’iso<strong>le</strong>r 160 patients sur une populationinitia<strong>le</strong> de 1600 individus.Parmi <strong>le</strong>s 160 patients, nous avons ensuite sé<strong>le</strong>ctionné <strong>le</strong>s patientsdont <strong>le</strong>s rendez-vous à venir s’étalaient de janvier 2010 à juin 2010,puis de janvier 2011 à juin 2011.Nous avons ainsi constitué deux groupes, <strong>le</strong> premier groupe estcomposé de sujets implantés bilatéral que l’on nommera ICIC toutau long de l’étude. Le second groupe de patients est implantécochléaire unilatéral et dispose d’une prothèse auditive controlatéra<strong>le</strong>,il sera nommé ICAA pour <strong>le</strong> reste de l’étude. Leur prothèseauditive est réglée par un audioprothésiste de <strong>le</strong>ur choix. Seuls 9patients ICIC et 8 patients ICAA ont participé à l’étude. Ce chiffre,apparemment peu é<strong>le</strong>vé au regard des 160 patients, tient au fait que<strong>le</strong>s patients ne fréquentent l’IFIC que pour <strong>le</strong>ur bilan annuel. Le faitque <strong>le</strong>s deux groupes n’aient pas la même tail<strong>le</strong> ne constitue pas unhandicap pour <strong>le</strong> traitement statistique.Nous avons vérifié que <strong>le</strong>s deux groupes constituent des échantillonsstatistiquement comparab<strong>le</strong>s. Notamment, nous avons veillé àéviter <strong>le</strong>s biais de sé<strong>le</strong>ction dans la constitution des groupes.3.3 Protoco<strong>le</strong>3.3.1 Données sur <strong>le</strong>s patientsLes informations cliniques et démographiques, l’âge del’implantation, <strong>le</strong> type d’implant, <strong>le</strong> type de pathologie, l’arrivée de lasurdité et <strong>le</strong> type de stratégie ont été extraits de la base de donnéesmidd<strong>le</strong>c@re.Après avoir recueilli l’ensemb<strong>le</strong> des informations et identifié <strong>le</strong>sdeux groupes de patients distincts, correspondants à nos critèresd’inclusion, chaque patient a été invité à remplir un questionnairespécifique.3.3.2 QuestionnaireLes questionnaires ont été développés spécia<strong>le</strong>ment pour l’étude.Chaque questionnaire est composé d’une partie propre à chaquegroupe (bimodal ou implanté bilatéral) et d’une partie communepermettant une comparaison.Pour <strong>le</strong>s patients implantés bilatéral <strong>le</strong>s questions spécifiquesportent sur :- l’utilisation effective d’une prothèse auditive controlatéra<strong>le</strong> avant laseconde implantation et sur la durée de son port,- <strong>le</strong>s raisons d’une seconde implantation,- <strong>le</strong>s différences ressenties après l’arrivée du second implantcochléaire.Pour <strong>le</strong>s patients bimodal <strong>le</strong>s questions spécifiques portent sur :- <strong>le</strong>s modalités de l’interaction entre l’aide auditive et l’implant,- <strong>le</strong> renouvel<strong>le</strong>ment de l’aide auditive depuis l’implantation,- la durée au bout de laquel<strong>le</strong> l’aide auditive a été portée aprèsl’implantation,- la nécessité de procéder à des réglages après l’implantation.Les questions communes aux deux groupes portent sur <strong>le</strong> type8 Les Cahiers de l’Audition - N°4/<strong>2012</strong>
