Ceprocessus aboutit certes à un résultatmais au prix d’un manque d’efficacitécertain, car il faut parfois plusieurs visitespour parvenir à des réglages subjectivementacceptab<strong>le</strong>s.Dans certains cas, <strong>le</strong> patient décide dese faire rembourser ses appareils sansattendre d’avoir obtenu une qualité sonoreoptima<strong>le</strong>.Ce document présente SoundPoint, unenouvel<strong>le</strong> méthode de réglage assisté par<strong>le</strong> patient qui complète <strong>le</strong> processus deréglage traditionnel et transfère une partiedu contrô<strong>le</strong> de la qualité sonore au patient.Alternative élégante à la méthode « entretien/réglage», ce système lui permet dese déplacer dans l’univers sensoriel desdifférentes possibilités de réglage de sesaides auditives.Sur un clavier ou à l’aide d’une souris, ilrèg<strong>le</strong> son appareil de manière intuitive,fluide et transparente et atteint la qualitésonore subjective qu’il préfère sans sesoucier de la comp<strong>le</strong>xité acoustique deses propres perceptions auditives.Les méthodes basées sur <strong>le</strong>s donnéesaudiométriques peuvent prescrire desgains très différents des réglages favorisdes patients (Jiejon et al., 1990) probab<strong>le</strong>mentparce que <strong>le</strong>s algorithmes nesont pas à même d’intégrer la variabilitéindividuel<strong>le</strong> en termes d’augmentationde la sonie, de niveau d’écoute <strong>le</strong> plusconfortab<strong>le</strong> (MCL) ou de niveau de confortsupérieur (UCL) car ils calcu<strong>le</strong>nt l’objectifde prescription à partir de la moyennedes données de test fournies par un vasteéchantillon d’auditeurs (Kiessling, 2001).Ainsi, Keidser et Dillon (2006) ont découvertque <strong>le</strong> niveau de confort d’écouted’une personne peut s’écarter jusqu’à 18dB des cib<strong>le</strong>s prescrites par NAL-NL1.La littérature médica<strong>le</strong> généra<strong>le</strong> suggèreque <strong>le</strong>s résultats thérapeutiques sontmeil<strong>le</strong>urs quand <strong>le</strong>s patients participent auprocessus de prise de décision.Cette observation se retrouve dans lalittérature sur <strong>le</strong>s aides auditives et nousrappel<strong>le</strong> que l’appareillage a de meil<strong>le</strong>ureschances de succès quand <strong>le</strong>s réglagessont conformes aux préférences individuel<strong>le</strong>sdes patients (Dillon et al., 2006).Ces résultats confirment l’avantage potentielde la personnalisation du réglage desaides auditives afin d’arriver à une qualitésonore acceptab<strong>le</strong> après la vérification del’audibilité adéquate des entrées voca<strong>le</strong>s.En bref, l’optimisation de la qualité sonoretenant compte des préférences individuel<strong>le</strong>sfait la différence entre équiper uneperte auditive et adapter une personne.SoundPoint - OriginesSoundPoint est un outil informatique detraitement des signaux développé pardes chercheurs des Laboratoires Starkey.Conçue comme un moyen rapide et simp<strong>le</strong>d’ajustement d’algorithmes comp<strong>le</strong>xes detraitement des signaux, cette interface deréglage des aides auditives assisté parl’auditeur permet à celui-ci de combiner64 paramètres en temps réel et quasimentinfini et de définir ses réglages préférés.La figure 1 représente une patiente entrain d’utiliser SoundPoint avec son iPad.Les réglages effectués avec SoundPointdépassant la simp<strong>le</strong> régulation du gain,son interface doit être plus qu’un simp<strong>le</strong>égaliseur graphique. C’est pourquoi<strong>le</strong> développement initial du système acherché à organiser l’espace de contrô<strong>le</strong>de manière à autoriser des changementsfluides et intuitifs de la qualité sonore. I<strong>le</strong>st rapidement apparu que, pour faciliterl’utilisation de l’interface aux auditeurs, ilfallait que l’espace de contrô<strong>le</strong> possèdeune cohérence sensoriel<strong>le</strong>. Le respect decette exigence nécessitait deux composantesmajeures.Tout d’abord, un mouvement dans unedirection donnée devait entraîner un changementcohérent d’une qualité sonoredonnée. Ensuite, un déplacement de petiteamplitude dans l’espace de contrô<strong>le</strong> devaitlégèrement modifier <strong>le</strong> son des aides auditiveset un déplacement de grande amplitudedevait donner un changement d’ordrede grandeur sensoriel supérieur.Résultat : quand <strong>le</strong>s patients se déplacentdans l’espace de contrô<strong>le</strong>, diversajustements sont fusionnés et appliquésaux aides auditives. L’organisation del’interface <strong>le</strong>ur permet de se déplacer versdes réglages favoris similaires sur <strong>le</strong> plansensoriel d’essai en essai, ce qui permetd’atteindre avec fiabilité la qualité sonoredésirée dans l’environnement SoundPoint.Des essais avec comparaison desmoyennes appariées ont révélé que <strong>le</strong>sutilisateurs préfèrent <strong>le</strong>s réglages de <strong>le</strong>ursaides auditives obtenus avec cet agencementorganisationnel optimisé plutôtqu’avec <strong>le</strong>s agencements par tri aléatoireet <strong>le</strong>s agencements autogénérés.Figure 1 : Une patiente définit <strong>le</strong>sréglages de ses aides auditives avecl’interface à distance de SoundPoint suriPad.Figure 2 : Écran de mode d’emploi deSoundPoint.
