Un beau menton
éQuilibre un visage !

obtenu lisse la peau et atténue également le pli de la lèvre inférieure.
On effectue 2 à 5 points d’injection profonds de part et d’autre de la
ligne médiane. Les suites sont simples. La durée d’action est d’environ
6 mois, un peu moins chez les fumeurs.
L’augmentation de volume d’un menton trop petit ou fuyant s’obtient
par injection volumatrice d’Acide Hyaluronique. Cette technique intéresse
les déformations modérées, les cas plus évolués étant réservés
à la chirurgie. L’injection s’effectue au centre du menton et doit
être faite progressivement et en profondeur pour venir soulever le
muscle. On redonne ainsi un galbe harmonieux sans sur-corriger afin
d’éviter un aspect boudiné peu naturel. Les suites sont simples, les
hématomes rares. La tenue du produit est d’environ 2 ans.
Les procédés chirurgicaux
La mentoplastie ou génioplastie, permet de corriger les anomalies
de position et de forme du menton. Ceci exclue de notre propos
la chirurgie associée des mâchoires, réservée aux cas complexes.
Elle est réalisée soit isolément, soit associée à une chirurgie du nez
(rhinoplastie) ou un lifting pour rééquilibrer le visage. Elle a un but
tant esthétique que fonctionnel pour remettre en contact les 2 lèvres.
Lors d’un lifting, elle améliore l’ovale du visage par son effet tenseur
sur la partie médiane du cou. L’intervention s’effectue par la bouche
sans cicatrice apparente.
Un menton fuyant se corrige de 2 manières :
l Soit en découpant une baguette osseuse sur le rebord de la mâchoire.
Cette baguette est remontée et fixée (vis, plaque métallique).
l Soit en utilisant un implant : matériau de synthèse, corail ou greffons
osseux prélevés sur le patient.
On peut aussi meuler simplement le rebord osseux du menton mais
le résultat sera plus modeste. A réserver aux progénies modérées.
Un pansement de maintien est porté plusieurs jours. Une anesthésie
transitoire de la lèvre inférieure est habituelle dans les suites. Inversement
pour corriger un menton trop proéminent, on diminue sa
hauteur en ôtant une bandelette osseuse horizontalement.
Quid du double menton ?
Il est dû à un excédent de tissu graisseux sous le menton.
Chez les sujets jeunes, la peau est simplement distendue. Une lipoaspiration
sous anesthésie locale sera suffisante. Les suites en général
sont simples.
A un âge plus avancé, il deviendra nécessaire de l’associer à un lifting
du cou pour retendre également la peau.
La radiofréquence peut constituer une alternative intéressante pour
retendre la peau du cou sans chirurgie. L’effet apparait en quelques
mois et peut perdurer plusieurs années.

The volumes of a too-small or weak chin can be increased by injecting
volumising hyaluronic acid. This technique is useful for treating
slight deformations; more marked deformations can only be resolved
through surgery. The injections are made in the centre of the chin and
must be carried out gradually and deeply in order to lift the muscle.
We thus achieve a harmonious shape without overcorrecting, so as
to avoid an unnatural bulging appearance. The after-effects are minor
and patients rarely suffer from bruising. The product used lasts
around 2 years.
Surgical procedures
Mentoplasty, or genioplasty, enables us to correct any anomalies in the
position and/or shape of the chin (this excludes surgery on the jaw,
which is only for extreme cases). It is carried out either on its own
or alongside nasal surgery (rhinoplasty) or a face lift, to rebalance
the face. It is carried out for both cosmetic and functional reasons, to
bring the two lips together.
A face lift improves the facial contours thanks to its tensing effect on
the median section of the neck. The operation is carried out through
the mouth and leaves no visible scars.
A weak chin can be corrected in one of two ways:
l By removing a small piece of bone from the edge of the jaw. This
piece of bone is then lifted and fixed (screw, metal plate).
l By fitting an implant: synthetic material, coral or bone grafts taken
from the patient.
We can also simply mould the bony edge of the chin, but the result is
not as noticeable so it should only be used for very slight prognathism.
A supportive bandage is to be worn for several days following surgery,
and feeling can temporarily be lost in the lower lip.
Conversely, to correct a too-prominent chin, we reduce its height by
removing a horizontal strip of bone.
What about double chins?
Double chins are caused by excess fatty tissue under the chin.
In younger patients, the skin is simply stretched. Liposuction carried
out under local anaesthetic is usually sufficient and the after-effects
are generally minor. In older patients, we must also lift the neck in
order to tighten up the skin.
Radio waves might be an interesting alternative for tightening up the
skin on the neck without surgery. The effects are noticeable after a
few months and can last for several years.
