Goal: Adults served by the Office of <strong>Mental</strong> <strong>Health</strong> will be provided with appropriate recovery/<br />

resiliency-oriented mental health services.<br />

Target: The average number of days between a consumer's discharge from a psychiatric hospital and a<br />

follow-up visit to a community mental health clinic (CMHC) will be at the lowest level possible<br />

in order to maintain continuity of care<br />

Population: Adults diagnosed with a Serious <strong>Mental</strong> Illness<br />

Criterion: 2: <strong>Mental</strong> <strong>Health</strong> System Data Epidemiology<br />

Indicator: Average number of days between a state psychiatric hospital discharge and a CMHC aftercare<br />

appointment<br />

Measure: Days reported<br />

Average = Number of days until follow-up divided by number of discharges<br />

Numerator = sum of days from discharge to CMHC admit<br />

Denominator = Discharges with aftercare visit within 45 days<br />

Time period (Lag fiscal year) - April 1 - March 31<br />

Information:<br />

Special Issues: This data now excludes data from all acute units. The numbers reported for 2008 have been<br />

adjusted to provide for accurate comparisons. In previous years reporting, the data included acute<br />

units within hospitals, because these numbers had not been separated out. At discharge, patients<br />

are routinely given 3 weeks supply of medications, so 21 days is the absolute limit for clients to<br />

be seen in the outpatient setting. Although this target was not technically met, the difference is<br />

very minor, and not particularly meaningful when comparisons are made between 8.5 days<br />

and 8.7 days. This target was set very conservatively at a maintenance level due to budgetary<br />

and workforce constraints, including layoffs of personnel and a hiring freeze. FY 2010 Actual<br />

= 575 / 66 = 8.7 (average)<br />

Significance: One of the strongest predictors of community success after discharge from a state hospital is<br />

continuity of care<br />

Action Plan: Efforts to decrease the number of days between discharge and follow-up aftercare will continue<br />

to be made, and should improve with the availability of more outpatient services. The <strong>Block</strong><br />

<strong>Grant</strong> indicators are monitored through the Committee on Programs and <strong>Services</strong> of the<br />

Louisiana <strong>Mental</strong> <strong>Health</strong> Planning Council. The Planning Council Committee on Programs and<br />

<strong>Services</strong> is responsible for monitoring and evaluation of the mental health system and for<br />

recommending service system improvements to the Council.<br />
