East Baton Rouge Parish Resource Fair provided resource information for transitional age<br />

youth<br />

The Transition Forum provides resource information to transitional age youth<br />

The Instructional resource Center provides resource information to parents and transitional<br />

youth<br />

Elm Grove Church provides information to transitional age youth and adults.<br />

Region III:<br />

Lafourche MHC - The child psychiatrist continues working with clients until they are able to<br />

receive services from an adult provider.<br />

St. Mary Transition Team - Manager sits on transition team which includes members from<br />

various agencies to assist those with disabilities leaving the school system<br />

Bayou Land Families Helping Families – family resources center that helps parents and<br />

children with transition services<br />

Federation of Families - Family liaison works with families to provide mentoring and<br />

educational guidance<br />

Region IV:<br />

CART - provides assistance to children and their families in times of crisis<br />

Region V:<br />

CMHC C/Y Units: Clinicians may see client up to age 21 if they are receiving special<br />

education services through CPSB, or up to age 19 if enrolled in school full time.<br />

ETC Housing-Transitional Housing program for transitional age youth<br />

Transitional Team Monthly meetings<br />

Region VI:<br />

FINS (Families in Need of <strong>Services</strong>) offers pre-court, legally sanctioned intervention for<br />

youth exhibiting anti-social behaviors.<br />

The Consumer Care Resource program assists children and families with meeting their basic<br />

needs.<br />

OMH Cottage Respite offers out of home planned respite services.<br />

“Whatever It Takes Program” assists families to obtain, coordinate, and advocate for<br />

needed services.<br />

Development and implementation of advanced training for CIT Law Enforcement Officers<br />

in the region on Juvenile <strong>Mental</strong> <strong>Health</strong> Issues.<br />

ISC (Interagency Service Coordination) links state agencies with community-based<br />

programs.<br />

Recreational Planned Respite offers planned recreational camp activities for youth and<br />

children. OMH Cottage Respite provides out of home planned respite services for children<br />

and youth.<br />

Region VII:<br />

Special Education Transition Team helps special education students connect with<br />

vocational services, trainings, and sheltered workshops.<br />

Co-occurring group focuses on topics specifically geared towards addressing substance use<br />

and recovery<br />
