PARENT INFORMED CONSENT FORM<br />
RESEARCH PROCEDURES<br />
This research is being conducted to study differences in teachers‘ book reading styles<br />
across classrooms and to test emotional st<strong>or</strong>ybooks f<strong>or</strong> social-emotional skills preschool<br />
curriculum development. This research is being funded by the American Educational<br />
Research Association and the U.S. Department <strong>of</strong> Education‘s Institute <strong>of</strong> Education<br />
Services. If you agree to participate, you will be asked to fill out two brief questionnaires.<br />
It is estimated that approximately 15 minutes will be required to complete these<br />
documents. If your child agrees to participate, he/she will complete two tasks with a<br />
researcher and will be observed by a researcher during the preschool day at two separate<br />
time points. In the tasks, your child will answer the researcher‘s questions using puppets<br />
and will answer questions the researcher asks about pictures. Each questionnaire your<br />
child completes should take about 15 minutes each, and the observations will not require<br />
any time outside <strong>of</strong> the classroom.<br />
Your child will also participate in book reading activities with his/her teacher 3 times per<br />
week f<strong>or</strong> approximately 12 weeks. Each activity should last about 20 minutes. These<br />
activities will be audio taped bef<strong>or</strong>e and at the beginning, the middle, and the end <strong>of</strong> the<br />
activity phase <strong>of</strong> the project to study differences in teachers‘ reading styles across<br />
classrooms. Certain aspects <strong>of</strong> the project cannot be discussed bef<strong>or</strong>e data collection, but<br />
you will be told about these at the end <strong>of</strong> the project.<br />
RISKS<br />
Potential risks to your child include negative feelings from talking about emotional<br />
st<strong>or</strong>ybooks with teachers. The researcher will talk to teachers on a regular basis to<br />
monit<strong>or</strong> your child‘s participation. If you think that your child is experiencing negative<br />
feelings from the study, let a researcher <strong>or</strong> teacher know right away. Your child will be<br />
removed from the study, and the researcher will talk with Head Start staff and<br />
administrat<strong>or</strong>s to be sure that your child can get help from Head Start‘s mental health<br />
services.<br />
BENEFITS<br />
Your child will get small-group academic attention from teachers in the study.<br />
CONFIDENTIALITY<br />
The data in this study will be confidential. Your name will not be written on the<br />
