A tertiary based retrospective case<br />
control study of the hospital<br />
computer records of all obstetric<br />
admissions to the ICU at the Royal<br />
Free Hospital London over a 10 year<br />
period (1993 to 2003). For this study,<br />
admissions from 14 weeks’ gestation<br />
up to 6 weeks postpartum were<br />
included. The wom<strong>en</strong> who delivered<br />
just before and after the indexed case<br />
were chos<strong>en</strong> as controls. The data<br />
collected included demographic data,<br />
pre-existing medical or surgical<br />
conditions, obstetric history,<br />
indications for, as well as the<br />
outcome of ICU admission. The<br />
duration of stay in the ICU, use of<br />
mechanical v<strong>en</strong>tilation, and c<strong>en</strong>tral<br />
and arterial monitoring were also<br />
noted.<br />

OBJECTIVE The objective was to review all obstetric<br />
admissions to the int<strong>en</strong>sive care unit (ICU) at the Royal<br />
Free Hospital, London, UK, and to id<strong>en</strong>tify the risk<br />
factors for obstetric admissions to the ICU.<br />
RESULTS: Thirty-three obstetric pati<strong>en</strong>ts were<br />
admitted to the ICU, repres<strong>en</strong>ting 0.11% of all deliveries.<br />
The ICU utilization rate was 0.81%. Eighty perc<strong>en</strong>t of the<br />
admissions were postpartum. The main indications for<br />
admission were hypert<strong>en</strong>sive disorders (39.4%), and<br />
obstetric haemorrhage (36.4%). There was no differ<strong>en</strong>ce<br />
betwe<strong>en</strong> cases and controls in, age, parity, smoking and<br />
employm<strong>en</strong>t status. Compared with controls, wom<strong>en</strong><br />
