3.3.4 Effici<strong>en</strong>cy (grey literature)<br />
• How are MIC-services financed?<br />
No international literature on MIC-financing was found. On a national level, since 1998,<br />
every hospital with accredited MIC-beds receives an extra amount of money to pay for<br />
the FTE of midwives with experi<strong>en</strong>ce in high-risk pregnancies. The additional MICbudget<br />
is worth 2.29 points (or 46.347,54€) per accredited MIC-bed. More details are<br />
found in the background-chapter.<br />
• How are they staffed?<br />
The international staffing-norm on services similar to the Belgian MIC-services is one<br />
midwife to two pati<strong>en</strong>ts, supervised by obstetric/gynaecologist specialist(s). The only<br />
published national data on staffing was found in a report of the Federal Belgian<br />
Governm<strong>en</strong>t from 2001. This report contains unfortunately only data on the<br />
paramedical staff on the Belgian Maternity-services: 81% are midwives, 7% graduated<br />
nurses, 7% qualified child care workers, 4% other health care staff and 1% vocational<br />
trained nurses. The distinction betwe<strong>en</strong> staff working in the MIC-services and in the<br />
Maternity-services is not made, they are included in the aforem<strong>en</strong>tioned perc<strong>en</strong>tages.<br />
No data on the medical personnel was found.<br />
Key Messages<br />
• The literature review alone did not result in a clear definition of maternal<br />
intermediate care, nor MIC-pati<strong>en</strong>t, nor MIC-service<br />
• The literature review resulted in very few evid<strong>en</strong>ce-based guidelines for<br />
evaluation and treatm<strong>en</strong>t that <strong>en</strong>tailed recomm<strong>en</strong>dations as to which level<br />
of care is indicated for various pregnancy-associated disorders<br />
• The working-definitions of MIC-pati<strong>en</strong>ts, <strong>Maternal</strong> Intermediate <strong>Care</strong> and<br />
the MIC-service were constructed by expert cons<strong>en</strong>sus based on the Belgian<br />
