Required Supplemental Materials<br />

The following supplemental materials are to be mailed directly to the <strong>KCUMB</strong><br />

admissions <strong>of</strong>fice:<br />

1. A completed <strong>KCUMB</strong> supplemental application.<br />

2. A non-refundable supplemental application fee <strong>of</strong> $50 made payable to<br />

<strong>KCUMB</strong>. (Fee is waived if applicant has received fee waiver for AACOMAS fees.)<br />

3. Letters <strong>of</strong> evaluation from each <strong>of</strong> the following sources are required:<br />

• A physician (preferably a D.O.).<br />

• A science faculty member who is familiar with the applicant’s academic<br />

work.<br />

• A premedical source. This could be an advisor’s letter, a composite<br />

evaluation or evaluations from a committee. If a premed source is not<br />

available, another science faculty, in addition to the above, or an employer’s<br />

letter may be substituted. Evaluations must be written within the two years<br />

prior to making applications. Example: If applying beginning May <strong>2009</strong>,<br />

letters must be dated no earlier than May 2007.<br />

• Applicants who anticipate making application for military scholarships<br />

should make arrangements to obtain additional copies <strong>of</strong> evaluation letters.<br />

Military scholarship committees require original letters for this purpose.<br />

Applicants anticipating applying for these scholarships should obtain<br />

additional evaluation letters from evaluators and have the letters placed in<br />

<strong>of</strong>ficially sealed envelopes.<br />

4. A signed and dated Technical Standards Certification. This form states that the<br />

applicant has read the Minimum Technical Standards and the Participation in<br />

Osteopathic Clinical Skills Laboratory. The completed form attests that the<br />

applicant can meet all requirements listed therein, either without<br />

accommodation or with reasonable accommodation from the <strong>University</strong>. The<br />

form does not ask for disclosure <strong>of</strong> disability.<br />

Applicants with college credit at foreign institutions, or institutions teaching in a<br />

language other than English, must submit <strong>of</strong>ficial foreign transcripts to one <strong>of</strong> the<br />

following evaluation services:<br />
