AND loss of interest or pleasure in usual activities
AND five or more of the following symptoms in the same 2 week period:
depressed mood most of the day, nearly daily
OR marked loss of interest and pleasure
OR significant weight loss (5% body weight in a month)
OR insomnia or hypersomnia almost daily
OR psychomotor agitation or retardation, almost daily
OR fatigue or loss of energy, nearly daily
OR feelings of worthlessness
OR feelings of excessive guilt
OR decreased ability to think
OR recurrent thoughts of death
