OR loss of interest or pleasure in usual activities
AND five (or more) of the following symptoms in the same 2 week period:
depressed mood most of day, nearly daily
OR marked loss of interest or pleasure
OR significant weight loss (5% body weight in a month)
OR insomnia or hypersomnia almost daily
OR psychomotor agitation or retardation, nearly daily
OR fatigue or loss of energy, nearly daily
OR feelings of worthlessness
OR feelings of excessive guilt
OR decreased ability to think
OR recurrent thoughts of death
AND one of the following symptoms:
impaired ability to maintain employment
OR impaired ability to care for self
