刘 倩 倩 黄 顺 梁 金 仙李 红 云 薛 雅 晋 孙 周解 放 军 第 302 医 院 内 科 ICU 北 京 100039摘 要目 的 : 研 究 重 症 监 护 病 房 (Intensive Care Unit,ICU) 髋 部 骨 折 患 者 术 后 谵 妄 的 护 理 ,提 出 护 理 对 策 。 方 法 : 分 析 107 例 术 后 转 入 I C U 髋 部 骨 折 患 者 , 用 I C U 意 识 紊 乱 评 估 方 法 (t h econfusion assessment method for the ICU,CAM-ICU) 进 行 监 测 ,38 例 (28.4%) 患 者 在 手 术后 7d 内 发 生 谵 妄 , 针 对 患 者 症 状 提 出 护 理 问 题 , 护 理 措 施 , 评 估 护 理 效 果 。 结 果 : 本 组 38 例患 者 , 术 后 恢 复 顺 利 , 均 于 术 后 1-7 天 转 出 I C U。 结 论 :I C U 髋 部 骨 折 患 者 手 术 后 谵 妄 发 生 率 较高 , 及 时 的 CAM-ICU 评 估 , 有 效 的 针 对 性 护 理 措 施 , 有 助 于 患 者 术 后 康 复 。关 键 词 :ICU 谵 妄 ; 髋 部 骨 折 ; 护 理责 任 作 者 与 联 系 方 式 : 刘 倩 倩 ,E-mail: 1278895765@qq.comICU 老 年 髋 部 骨 折 患 者 术 后 谵 妄 38 例 的 护 理Nursing of Delirium in 38 Aged Patients with Hip Fracture after Operation in ICUQian-qian Liu, Shun Huang, Jin-xian Liang, Hong-yun Li, Ya-jin Xue, Zhou SunDepartment of ICU, the 302th Hospital of Chinese People’s Liberation Army, Beijing, 100039AbstractObjective: To investigate the nursing of the delirium patients with hip fracture after operation in ICU and put forward the nursing strategy.Methods: Analyzing the 107 patients with hip fracture who were moved to ICU after operation and monitoring the patients by the confusionassessment method for the ICU (CAM-ICU). 38 patients (28.4%) had delirium 7days after operation. To raise nursing questions and nursing measuresbased on the patients’ symptoms, and then to evaluate the nursing effect.Results: In the group, 38 patients recovered smoothly and were transferred out ICU 1-7days after operation.Conclusion: The occurrence rate of delirium in ICU patients with hip fracture after operation is high. Timely CAM-ICU evaluation and effectivespecific nursing measures are helpful for the recovery of patients.Key Words: ICU delirium; Hip fracture; NursingCorresponding Author: Qian-qian Liu, E-mail: 1278895765@qq.com谵 妄 是 表 现 为 意 识 、 注 意 力 、 感 知 觉 、 思 维 、 记 忆 、 行为 、 情 绪 障 碍 和 睡 眠 - 觉 醒 周 期 功 能 紊 乱 的 一 组 病 因 非 特 异性 的 综 合 症 [1] 。 是 髋 部 骨 折 患 者 术 后 最 常 见 的 并 发 症 。 有 研究 报 道 , 术 后 谵 妄 的 病 人 死 亡 率 会 增 加 [2] 。2008 年 1 月 至 2011年 10 月 107 例 髋 部 骨 折 手 术 后 患 者 入 住 I C U, 其 中 38 例 出 现 谵妄 症 状 。 现 总 结 护 理 措 施 , 报 告 如 下 。一 、 临 床 资 料1. 一 般 资 料2008 年 1 月 至 2011 年 10 月 共 107 例 髋 部 骨 折 手 术 后 入 住 ICU患 者 , 麻 醉 方 式 均 为 全 麻 。 用 I C U 意 识 紊 乱 评 估 方 法 (t h econfusion assessment method for the ICU,CAM-ICU) [3-4]进 行 监 测 , 诊 断 为 谵 妄 者 38 例 , 其 中 男 性 22 例 , 女 性 16 例 ,年 龄 68-92 岁 , 平 均 (76.3 士 7.4) 岁 。 谵 妄 发 生 率 28.4%。 均为 活 跃 型 谵 妄 。 患 者 的 主 要 表 现 为 : 思 维 不 连 贯 , 有 时 胡 言乱 语 , 烦 躁 不 安 , 情 绪 激 动 , 有 幻 听 和 幻 视 , 拒 绝 治 疗 和 护理 , 行 为 过 激 , 意 欲 拔 出 各 种 管 路 , 谩 骂 医 护 人 员 。2. 结 果诊 断 为 谵 妄 的 38 例 患 者 , 有 痴 呆 史 24 例 , 占 63.2%, 血管 危 险 因 素 评 估 ≥3 的 30 例 , 占 78.9%, 血 红 蛋 白
