<strong>30</strong> September <strong>20</strong>11 <strong>TO</strong> <strong>00</strong>-<strong>20</strong>-<strong>14</strong>5.13 AF<strong>TO</strong> FORM 57, PMEL TECHNICAL ORDER INFORMATION.The AF<strong>TO</strong> Form 57 (see Figure 5-11) is for the PMEL to record data relative to a calibration procedure that pertains totheir peculiar situation. It shall be filed in the folder holding the pertinent calibration <strong>TO</strong>. This form is not required witheach <strong>TO</strong>. It is to be completed to the degree (and when) PMEL personnel feel is necessary or as needed. Also, it shallnot be used in place of AF<strong>TO</strong> Form 22 action. The following guidance is provided for entries on this form:NOTEThe technician shall verify information entered on the AF<strong>TO</strong> Form 57 prior to usingsuch information.a. Block 1, MAINTENANCE <strong>TO</strong> INFORMATION. Enter the part number and maintenance <strong>TO</strong> for the differentmodels or part numbers covered by the procedure in the appropriate blocks.b. Block 2, ADDITIONAL ITEMS REQUIRED FROM THE OWC. Enter any additional ancillary items orequipment that must be supplied by the OWC to facilitate repair or calibration of the TMDE entered in Block 1.c. Block 3, LOCATION OF TEST FIXTURES. Enter the PMEL storage location of test fixtures required forrepair or calibration of the TMDE entered in Block 1.d. Block 4, SUITABLE SUBSTITUTES FOR STANDARDS REQUIRED IN CALIBRATION PROCEDURES.Enter the part number of all TMDE listed in the procedure that is not locally available and the part number of thesubstitute standard in the appropriate blocks.NOTEEnsure required uncertainty ratios are maintained when substituting standards duringcalibration.e. Block 5, AF<strong>TO</strong> FORM 22 INFORMATION. Enter all AF<strong>TO</strong> Form 22 information as necessary to identifydeficiencies in the procedure. Enter date submitted, paragraph affected, and name of technician submitting theAF<strong>TO</strong> Form 22 as a minimum.NOTEApproval of an AF<strong>TO</strong> Form 22 is not justification to deviate from published technicaldata IAW <strong>TO</strong> <strong>00</strong>-5-1.f. Block 6, ALTERNATE PROCEDURE INFORMATION. Enter any information that shall allow the technicianto locate or perform approved alternate procedures.g. Block 7, PECULIARITIES OF TMDE OR CALIBRATION PROCEDURE. Enter any information that willassist the technician during calibration of the TMDE.h. Block 8, HIGH FAILURE COMPONENTS. Enter any components that are considered high failure items orcomponents that are difficult to identify (Part Number and NSN) in the appropriate blocks.5.13.1 Continuation Sheets. Continuation sheets can be attached as necessary.5-15
