Purpose. To inform the Director, Directorate of Logistics (DOL) of the accident profile and<br />
trend analysis regarding DOL’s fleet of 5-ton commercial trucks (DeLorean 500).<br />
Discussion Points.<br />
· DOL’s truck fleet (DeLorean 500) experienced a significant rise in vehicle accidents over<br />
the last two quarters (2 d and 3 d quarters, FYxx).<br />
· Vehicle accidents and mechanical failures increased 54 percent and 73 percent,<br />
respectively, since 1 April 20xx.<br />
· During 3 d quarter, FYxx, DOL’s 5-ton truck fleet experienced 13 accidents and 18<br />
mechanical failures (that resulted in damages), accounting for 46 percent of Fort Riley’s<br />
total vehicle accidents and safety related incidents last quarter.<br />
- Final drive shaft failure was the largest contributor to accidents and mechanical<br />
failures.<br />
ü Final drive shafts are directly attributable to 85 percent of the accidents involving<br />
the DOL truck fleet (11/13).<br />
ü Final drive shaft failures account for over 83 percent of the mechanical failures<br />
(15/18).<br />
- The DeLorean 500 fleet is accountable for 5 loss days due to personal injuries and<br />
over $150,345 due to damages during accidents or mechanical failure.<br />
Assessments.<br />
· DOL’s DeLorean 500 fleet is experiencing an unprecedented increase in accidents and<br />
mechanical failures directly related to the final drives.<br />
· The DeLorean 500 fleet is a safety risk to its users and to the Fort Riley community.<br />
Recommendation. Purchase new trucks or stand down the 5-ton fleet to inspect and rebuild<br />
the existing trucks.
