(AO 23 B3 - C3) fracture patterns. To date, 1 delayed union and 1 patient with a Chronic Regional<br />
Pa<strong>in</strong> Syndrome were recorded. One patient had a secondary loss of reduction due to <strong>in</strong>sufficient fragment<br />
fixation. Compared with the un<strong>in</strong>jured contra-lateral wrist, average range of motion reached 77%<br />
at 6-week follow-up (ROM wrist: Flexion 76%; extension 69%, radial abduction 70%, ulnar abduction<br />
74%, pronation 96%, sup<strong>in</strong>ation 78%). At 1-year follow-up, our study group presented with a good to<br />
excellent outcome regard<strong>in</strong>g PRWE (median 3, range 0-8), ROM (average 94%), grip strength and the<br />
Lidstrom Score as well. At the time of the study presentation, an expected total of 50 patients will have<br />
completed their 1-year follow-up evaluation.<br />
Conclusion: Accord<strong>in</strong>g to our experience, the variable lock<strong>in</strong>g technology with the new VA-LCP is a<br />
reliable implant provid<strong>in</strong>g very good results concern<strong>in</strong>g patient satisfaction, functional and radiological<br />
outcome. Even <strong>in</strong> elderly patient with poor bone quality and/or complex fracture patterns a reliable and<br />
stable fixation can be achieved allow<strong>in</strong>g early functional treatment after surgery.<br />

M<strong>in</strong>imal <strong>in</strong>vasive long PHILOS ® -plate osteosynthesis <strong>in</strong> metadiaphyseal fractures of the proximal humerus<br />

Objective: M<strong>in</strong>imal <strong>in</strong>vasive plate osteosynthesis (MIPO) should belong nowadays to the armentarium<br />
of each trauma surgeon. MIPO not only meets the criteria of a “biological” osteosynthesis by m<strong>in</strong>imiz<strong>in</strong>g<br />
<strong>in</strong>vasivity as well as iatrogenic soft tissue damage caused by the operation, but can also provide<br />
adequate reduction and stability for fracture heal<strong>in</strong>g and early functional aftertreatment. Up to date,<br />
only few publications report on MIPO of humeral shaft fractures ma<strong>in</strong>ly us<strong>in</strong>g the antero-lateral deltopectoral<br />
approach for plate <strong>in</strong>sertion. Objective of the present study is assess the feasibility and cl<strong>in</strong>ical<br />
outcome of MIPO for metadiaphyseal fractures of the proximal humerus through a lateral approach<br />
us<strong>in</strong>g long PHILOS ® -plates.<br />
Methods: We retrospectively evaluated 29 patients (mean age 77 years, range 48-95) with displaced<br />
metadiaphyseal fractures of the proximal humerus treated <strong>in</strong> MIPO technique us<strong>in</strong>g an angular stable<br />
long PHILOS ® -plate. A lateral deltoid-split approach was used proximally and a brachialis/ brachioradialis<br />
<strong>in</strong>termuscular approach with exposure of the radial nerve was used distally. There were 23 acute<br />
fractures <strong>in</strong>clud<strong>in</strong>g two periprosthetic as well as one pathological fracture. Three patients were operated<br />
after failed conservative treatment, one for delayed-union and two cases were revision surgeries.<br />
Results: There were no <strong>in</strong>fections and no iatrogenic <strong>in</strong>juries to the axillary and radial nerve, respectively.<br />
All the 29 patients were immediately allowed active shoulder and elbow movement. One patient had to<br />
be reoperated ten weeks postoperatively for redislocation of the distal fragment with screw breakage,<br />
which was most likely due to <strong>in</strong>correct screw placement. This patient was successfully operated us<strong>in</strong>g<br />
the same method and implant. Whereas one patient refused follow-up, 28 patients showed entirely<br />
healed fractures and satisfactory shoulder and elbow function after a mean follow-up of 8 months<br />
(range 3 - 12 months).<br />
Conclusion: M<strong>in</strong>imal <strong>in</strong>vasive long PHILOS ® -plate osteosynthesis us<strong>in</strong>g a comb<strong>in</strong>ed lateral deltoid-split<br />
and brachialis/brachioradialis <strong>in</strong>termuscular approach proved to be a safe and viable procedure for<br />
the treatment of metadiayphyseal fractures of the proximal humerus with low morbidity and good functional<br />
outcome.<br />

M<strong>in</strong>imal-<strong>in</strong>vasive percutane Stabilisation proximaler Humerusfrakturen mit dem Button Fix System<br />

Objective: Das Button Fix System stellt e<strong>in</strong> neues m<strong>in</strong>imal-<strong>in</strong>vasives Stabilisierungssystem zur Vers<strong>org</strong>ung<br />
proximaler Humerusfrakturen dar. Es besteht aus e<strong>in</strong>er PEEK Platte mit 4 Löchern, welche<br />
percutan auf den lateralen proximalen Humerus aufgesetzt wird. Durch die Platte werden 4 Kirschner<br />
Drähte w<strong>in</strong>kelstabil <strong>in</strong> den Humeruskopf e<strong>in</strong>gebracht und dieser so fixiert. Ziel der Studie war die<br />
Evalua tion des kl<strong>in</strong>ischen und radiologischen Outomes nach m<strong>in</strong>imal-<strong>in</strong>vasiver Osteosynthese proximaler<br />
Humerusfrakturen mit dem Button Fix.<br />
Methods: Zwischen 2004 und 2006 wurden 18 Patienten (7 Männer, 11 Frauen) mit proximalen<br />
Humerusfrakturen mit dem Button Fix System vers<strong>org</strong>t. E<strong>in</strong>schlusskriterium war die Verwendung des<br />
Button Fix. Pathologische Frakturen wurden von der Studie ausgeschlossen. Das Durchschnittsalter<br />
der Patienten zum Zeitpunkt der OP betrug 66 Jahre. 13 Patienten (72%) konnten nach e<strong>in</strong>em durchschnittlichen<br />
Follow-up von 18 Monaten kl<strong>in</strong>isch und radiologisch nachuntersucht werden. Im Rahmen<br />
der letzten Nachuntersuchung wurde zusätzlich der Constant-Murrley Score, der DASH Score sowie<br />
der SF36 erhoben.<br />
Results: Postoperativ fand sich bei 13 Patienten e<strong>in</strong> anatomischer radiologischer Kopf/Schaft W<strong>in</strong>kel,<br />
4 Patienten zeigten e<strong>in</strong>e leichte Valgus Dislokation und 1 Patient e<strong>in</strong>en leicht varischen Kopf/Schaft<br />
W<strong>in</strong>kel. In e<strong>in</strong>em Fall kam es zu e<strong>in</strong>er sekundären Frakturdislokation, so dass e<strong>in</strong>e offene Revision<br />
mittel PHILOS Platte notwendig wurde. Bei den verbleibenden 17 Patienten wurde 6 Wochen postoperativ<br />
e<strong>in</strong>e Metallentfernung durchgeführt. Zum Zeitpunkt der letzten Nachuntersuchung fand sich<br />
e<strong>in</strong>e durchschnittliche Beweglichkeit der operierten Schulter von: Flex/Ext: 135°/0/45°, Abd: 142°,<br />
Iro/Aro: 51°/0/62°. Der durchschnittliche Constant-Murrley Score betrug 70 Punkte und der DASH<br />
Score 23 Punkte. Die Patienten erzielten des Weiteren e<strong>in</strong>en mittleren SF36 von 78 Punkten. E<strong>in</strong> Patient<br />
zeigte radiologische Zeichen e<strong>in</strong>er avaskuären Nekrose. Der Patient war zufrieden mit dem Behandlungsergebnis<br />
und lehnte e<strong>in</strong>e Revisionsoperation ab.<br />
Conclusion: Das Button Fix System stellt e<strong>in</strong>e valide m<strong>in</strong>imal-<strong>in</strong>vasive Behandlungsoption <strong>in</strong> der<br />
Therapie ausgewählter proximaler Humerusfrakturen dar. Die Komplikationsrate ist im Vergleich zu<br />
v<strong>org</strong>ängigen percutanen Verfahren ger<strong>in</strong>g. E<strong>in</strong>e adäquate Frakturstabilisation sche<strong>in</strong>t auch bei älteren<br />
Patienten mit potentiell osteopenem Knochen möglich.<br />

8.4<br />
Management of fractures of the coronoid process <strong>in</strong> acute elbow dislocation<br />

Objective: Biomechanical studies and cl<strong>in</strong>ical observation have shown the importance of the coronoid<br />
process <strong>in</strong> the stability of the elbow. Especially <strong>in</strong> elbow dislocation correct management of fractures of<br />
the coronoid process of the ulna is essential to restore a normal elbow function. Because of the variety<br />
of <strong>in</strong>jury pattern a differenciated management concept ist the key to succesfull treatment. We present<br />
our concept and patient outcomes.<br />
Methods: A series of 13 fractures of the coronoid process of the ulna <strong>in</strong> 13 patients (4 female, 9 male)<br />
mean age 41 years (21 to 67) was reviewed. All patients presented with acute elbow dislocation. The<br />
Regan-Morrey classification, treatment, associated <strong>in</strong>juries, course and outcome were evaluated.<br />
Results: Of the 13 fractures, 2 were type-I, 6 type-II and 5 type-III. All fractures where analysed by CT-<br />
Scan. Approach was performed from the radial side (6 patients), the ulnar side (3), bilateral (3) and<br />
from ventraly <strong>in</strong> 1 case. In 7 patients fracture was treated with screw-fixation. 6 where treated by transosseous<br />
sutures placed through drill-holes. 7 patients had a associated <strong>in</strong>jury of the collateral ligaments<br />
or the radial head to be fixed additionally. At the end of follow up after 344 days (median 150 to<br />
366) elbow flexion/extension showed a mean value of 135/4/0. Patients returned fully to work after<br />
45 days (median 30-170).<br />
Conclusion: Cl<strong>in</strong>ical exam<strong>in</strong>ation of elbow stability and CT Scan are essential for the <strong>in</strong>dication of operative<br />
treatment of fractures of the coronoid process of the ulna. In case of <strong>in</strong>stability the coronoid<br />
process should be fixed by screws or transosseous suture depend<strong>in</strong>g on size and shape of the fracture.<br />
Associated <strong>in</strong>jurys of collateral ligaments or the radial head can determ<strong>in</strong>e the approach to the<br />
coronoid process. With a differenciated concept it is possible, even <strong>in</strong> type-III fractures to achieve good<br />
functional results regard<strong>in</strong>g Range of Motion and return to work.<br />

M<strong>in</strong>imal-<strong>in</strong>vasive percutane Osteosynthese von Humerusschaftfrakturen mittels langer PHILOS Platte<br />

Objective: Die percutane m<strong>in</strong>imal-<strong>in</strong>vasive Plattenosteosynthese (MIPO) hat <strong>in</strong> der Behandlung von<br />
Humerusschaftfrakturen <strong>in</strong> den letzten Jahren zunehmend an Bedeutung gewonnen. Diese Methode<br />
wird <strong>in</strong> unserer Abteilung seit 2005 mit Erfolg angewendet. Ziel der vorliegenden Studie ist die kl<strong>in</strong>ische<br />
und radiologische Evaluation der Behandlungsergebnisse nach m<strong>in</strong>imal-<strong>in</strong>vasiver Osteosynthese von<br />
Humerusschaftfrakturen mittels langer PHILOS-Platte.<br />
Methods: Zwischen 2005 und 2009 wurden 16 Patienten mit Humerusschaftfraktur mittels m<strong>in</strong>imal<strong>in</strong>vasiver<br />
PHILOS Plattenosteosynthese über e<strong>in</strong>en re<strong>in</strong> anterioren Zugang oder e<strong>in</strong>en Delta-Split<br />
Zugang vers<strong>org</strong>t. E<strong>in</strong>schlusskriterium war die MIPO e<strong>in</strong>er Humersuschaftfraktur, die mittels langer<br />
PHILOS Platte ver<strong>org</strong>t wurde. Pathologische Frakturen sowie juvenile Frakturen wurden von der Studie<br />
ausgeschlossen. 13 Patienten (81%) konnten im Durchschnitt 2,6 Jahre postoperativ kl<strong>in</strong>isch und radiologisch<br />
nachuntersucht werden. Im Rahmen der letzten Nachuntersuchung wurde zusätzlich der<br />
Constant-Murrley Score, der DASH Score sowie der SF36 erhoben.<br />
Results: In e<strong>in</strong>em Fall kam es postoperativ zu e<strong>in</strong>em proximalen Ausbruch der Platte was e<strong>in</strong>e offene<br />
Re-Osteosynthese erforderte. E<strong>in</strong>e Patient<strong>in</strong> hatte e<strong>in</strong>e transiente Parästhesie im Ausbreitungsgebiet<br />
des N. cutaneus antebracchii. Bei e<strong>in</strong>em Patienten zeigte sich 1 Jahr postoperativ e<strong>in</strong>e Pseudarthrose<br />
mit Implantatversagen, so dass e<strong>in</strong>e offene Re-Osteosynthese mittels Metaphysenplatte und<br />
Spongiosa plastik durchgeführt wurde. In 2 Fällen war aufgrund e<strong>in</strong>es subacromialen Imp<strong>in</strong>gements<br />
durch die Platte e<strong>in</strong>e Metallentfernung notwendig. Zum Zeitpunkt des letzten Follow-up fanden sich<br />
gute durchschnittliche Constant-Murrley Score, DASH und SF36 Werte. Läsionen des N. radialis wurden<br />
weder prä- noch postoperativ beobachtet.<br />
Conclusion: Die m<strong>in</strong>imal <strong>in</strong>vasive Plattenosteosynthese mittels langer PHILOS Platte stellt bei<br />
entsprechender Indikation e<strong>in</strong> effektives und sicheres Verfahren zur Behandlung von Humerusschaftfrakturen<br />
dar. Sie erlaubt e<strong>in</strong>e stabile Frakturvers<strong>org</strong>ung mit vergleichbarer Komplikationsrate wie<br />
offene Verfahren.<br />

Utiliz<strong>in</strong>g lock<strong>in</strong>g head screws <strong>in</strong> the diaphysis m<strong>in</strong>imizes the amount of secondary radial shorten<strong>in</strong>g<br />
after volar plate fixation of distal radius fractures<br />

Objective: Fractures of the distal radius are common and volar plate fixation has become a widely<br />
used technique to achieve anatomical restoration. The advantage to utilize angular stable screws <strong>in</strong> the<br />
distal part of such plates seems obvious because of the mellow bone quality <strong>in</strong> the epi-/metaphyseal<br />
region. It is unclear however if the use of these expensive lock<strong>in</strong>g head screws for the fixation of the<br />
plate <strong>in</strong> the diaphyseal bone is superior to conventional screw fixation <strong>in</strong> the cl<strong>in</strong>ical sett<strong>in</strong>g.<br />
Methods: Between 01.01.2006 and 30.06.2008 we treated 68 patients with isolated distal radial fractures<br />
by open reduction and <strong>in</strong>ternal fixation with volar lock<strong>in</strong>g plates. The type of proximal fixation of<br />
the plate utiliz<strong>in</strong>g either conventional cortical screws or lock<strong>in</strong>g head screws was left to the discretion<br />
of the surgeon. X-rays of the wrist were taken a few days postoperatively and one year postoperatively.<br />
Radial shorten<strong>in</strong>g (RS) over this one year period was determ<strong>in</strong>ed us<strong>in</strong>g the method of Warwick. Statistic<br />
analysis was performed us<strong>in</strong>g Mann-Whitney-U-test.<br />
Results: 60 patients were available for control 1 year postoperative. Accord<strong>in</strong>g to the AO classification<br />
fractures were classified as 23-A <strong>in</strong> 29, 23-B <strong>in</strong> 10 and 23-C <strong>in</strong> 21 patients. For the measurement of<br />
RS only A and C fractures were <strong>in</strong>cluded. Overall RS was 0.7 +- 0.9 mm. Fractures were divided <strong>in</strong>to<br />
fractures with or without comm<strong>in</strong>ution zone <strong>in</strong> the metaphyseal area. In fractures without comm<strong>in</strong>ution<br />
(n=17) no difference of the amount of RS was observed regard<strong>in</strong>g the type of proximal fixation<br />
(conventional: n=10, RS=0.4 +- 0.8 mm; lock<strong>in</strong>g: n=7, RS=0.5 +- 0.8 mm). In fractures with comm<strong>in</strong>ution<br />
(n=33) there was significantly less shorten<strong>in</strong>g <strong>in</strong> the lock<strong>in</strong>g group compared to the conventional
