3-D endo-stomal ultrasound ( 3-DESUS )<br />

Objective: The utility of the 3-DESUS for stoma evaluation has never been described <strong>in</strong> <strong>in</strong>ternational<br />
literature.We believe that 3-DESUS is a reliable approach for early diagnosis of colostomy<br />
disorders(<strong>in</strong>cidence of a major problem 50%).We want to demonstrate feasibility/utility of 3-DESUS <strong>in</strong><br />
early detection of para-stomal disorders.<br />
Methods: Prospective double bl<strong>in</strong>d randomized study from 01.08.08 to 01.08.09. Selection of 50<br />
consecutive patients with a colostomy. 25 underwent a cl<strong>in</strong>ic evaluation, physic exam<strong>in</strong>ation and a<br />
3-DESUS (360°BK-2050 rigid probe,B-K Medical,Denmark);other 25 patients underwent cl<strong>in</strong>ic evaluation,<br />
physic exam<strong>in</strong>ation and abdom<strong>in</strong>al-wall ultrasound (AWUS), 7.5 MHz Aloka SSD-1700 Japan).<br />
Risk factors <strong>in</strong>cluded: age,body mass <strong>in</strong>dex, pre-op. sitt<strong>in</strong>g, size, abdom<strong>in</strong>al wall thikness, experiece of<br />
surgeon, emergency/elective procedure, diabetes, type of stoma and suture material. The type/<strong>in</strong>dications<br />
for surgery were also recorded. The complications were documented and a photographic record<br />
taken. Statistical analysis used univariate and multivariate methods performed by SPSS 10.<br />
Results: Mean age 64 years (std.deviation 16.01, range 26-79). Mean body mass <strong>in</strong>dex 34.5 (std.<br />
deviation 4.66, range 25-41). A comparative evaluation found 100% accuracy made by qualified coloproctologist<br />
on the early detection of para-stomal hernias (n=11) and small abscess (n=1) apply<strong>in</strong>g<br />
3-DESUS probe. In one case the AWUS had missed/missevaluated <strong>in</strong> a 39 BMI patient a para-stomal<br />
hernia (p>0.001) and a 24mm para-stomal abscess (p>0.001) <strong>in</strong> a patient with meteorism.<br />
Conclusion: 3-DESUS performed by experienced exam<strong>in</strong>ators is useful and precise <strong>in</strong> early detection<br />
of hernia and small abscess. Reproducibility makes the procedure sure and reliable. Pitfalls due to<br />
exam<strong>in</strong>ators are comperable than a rout<strong>in</strong>e AWUS. Otherwise, AWUS has several limitations <strong>in</strong> case of<br />
obesity or meteorism which is not the case for the 3-DESUS. Variability and other results are discussed.<br />

Anastomose Colo-anale. Quelle qualité de vie?<br />

Objective: Évaluer, à l’aide de formulaires standardisés, les résultats péri-opératoires et la qualité de vie<br />
post opératoire des reconstructions colo-anales.<br />
Methods: De 2003 à 2008, 105 patients ont subis une résection antérieure basse avec anastomose<br />
colo-anale et stomie de protection. 102 opérations ont été effectuées pour un Carc<strong>in</strong>ome du rectum,<br />
deux pour une fistule recto-vag<strong>in</strong>ale après résection sigmoïdienne et une pour sténose rectale après<br />
résection sigmoïdienne. La morbidité péri-opératoire a été relevée de façon prospective. Le contrôle<br />
à long terme a été effectué à l’aide des formulaires standardisés selon Eypasch(GIQLI) et Wexner.<br />
Results: Le collectif comportait 41 femmes et 54 hommes. Chez 35 patients, un réservoir comme<br />
remplacement du rectum a été confectionné; chez 68 patients, seule une anastomose latéro-term<strong>in</strong>ale<br />
a été effectuée. La durée opératoire était de 250 m<strong>in</strong> pour les 2 types de reconstructions. La morbidité<br />
chirurgicale était de 3%: 2 patients ont présentés une fuite anastomotique, l’une d’elle ayant<br />
nécessité une amputation et 1 patient à dû être réopéré en raison d’une ischémie colique. La durée<br />
d’hospitalisation était de 17 jours. La létalité a été nulle. Tous les patients encore en vie à l’heure<br />
actuelle ont reçu le formulaire de contrôle avec un taux de retour de 77%. La durée moyenne entre<br />
l’opération et le contrôle était de 2,1 ans. Les scores d’Eypasch et de Wexner étaient comparables pour<br />
les deux techniques: 110 (Eypasch) et 8 (Wexner).<br />
Conclusion: L’anastomose colo-anale avec stomie de protection est une technique sûre. La morbidité<br />
chirurgicale est faible. Toutefois, la qualité de vie et le score d’<strong>in</strong>cont<strong>in</strong>ence, mesurés à 2 ans, sont<br />
clairement plus faibles que dans une population de référence. Une bonne <strong>in</strong>formation des patients est<br />
donc <strong>in</strong>dispensable et une stomie déf<strong>in</strong>itive devra toujours être discutée.<br />

Effects of laparoscopic sleeve gastrectomy on lipid profile<br />

Objective: Determ<strong>in</strong>e the impact of sleeve gastrectomy on lipid profile<br />
Methods: Patients who underwent sleeve gastrectomy <strong>in</strong> our tertiary care hospital <strong>in</strong> the period<br />
from January and July 2007 were retrospectively reviewed. Lipid profile (serum LDL-c, HDL-c,T-<br />
Cholesterol[TC], Triglycerides[TG]) were compared preoperatively and one year post surgery.<br />
Results: Total of 103 patients were reviewed,48 patients were <strong>in</strong>cluded, while 55 excluded due to luck<br />
of totally exhaustive laboratory data. Statistical analysis were performed with SPSS 10.0 (SPSS, Chicago,<br />
Il, USA). Mean preoperative LDL-C, HDL-c, TC, TG were 2.81+/- 0.86, 1.37+/-0.32, 4.90+/- 1.05,<br />
