(iii) Significant change in status assessment.<br />

(iv) Significant correction <strong>of</strong> prior full assessment.<br />

(v) Significant correction <strong>of</strong> prior quarterly assessment.<br />

(vi) Quarterly review.<br />

(vii) A subset <strong>of</strong> items upon a resident’s transfer, reentry, discharge, <strong>and</strong> death.<br />

(viii) Background (face-sheet) information, for an initial transmission <strong>of</strong> MDS<br />

data on a resident that does not have an admission assessment.<br />

Intent: §483.20(f)(3):<br />

Facilities are required to electronically transmit MDS data to the <strong>CMS</strong> <strong>System</strong> for each<br />

resident in the facility. The <strong>CMS</strong> <strong>System</strong> for MDS data is named the QIES ASAP <strong>System</strong>.<br />

Interpretive Guidelines §483.20(f)(3):<br />

Background (face-sheet) information refers to the MDS Entry tracking record, while the<br />

discharge subset <strong>of</strong> items refers to the MDS Discharge assessment.<br />

§483.20(f)(4) Data format. The facility must transmit data in the format specified<br />

by <strong>CMS</strong> or, for a State which has an alternate RAI approved by <strong>CMS</strong>, in the format<br />

specified by the State <strong>and</strong> approved by <strong>CMS</strong>.<br />

Intent §483.20(f)(1-4):<br />

The intent is to enable a facility to better monitor a resident‘s decline <strong>and</strong> progress over<br />

time. Computer-aided data analysis facilitates a more efficient, comprehensive <strong>and</strong><br />

sophisticated review <strong>of</strong> health data. The primary purpose <strong>of</strong> maintaining the assessment<br />

data is so a facility can monitor resident progress over time. The information should be<br />

readily available at all times.<br />

Interpretive Guidelines §483.20(f)(1-4):<br />

―Encoding‖ means entering MDS information into a computer.
