Using universal precautions <strong>and</strong> clean technique <strong>and</strong> as per<br />

facility/manufacturer‘s directions when stopping, starting, flushing, <strong>and</strong> giving<br />

medications through the tube;<br />

Using hang time recommendations by the manufacturer to prevent excessive<br />

microbial growth;<br />

Implement the procedures to ensure cleanliness <strong>of</strong> supplies, e.g. irrigating<br />

syringes changed on a regular bases as per facility policy. It is not necessary<br />

to change the irrigating syringe each time it is used;<br />

Using a pump equipped with a functional alarm (if pump used);<br />

The facility‘s criteria for determining that a resident may be able to return to<br />

eating by mouth (e.g., a resident whose Parkinson‘s symptoms have been<br />

controlled);<br />

There are sampled residents meet these criteria;<br />

If so, the facility has assisted them in returning to normal eating; <strong>and</strong><br />

Identify if resident triggers CAAs for feeding tubes, nutritional status, <strong>and</strong><br />

dehydration/fluid maintenance. Consider whether the CAA process was used<br />

to assess causal factors for decline, potential for decline <strong>and</strong> lack <strong>of</strong><br />

improvement.<br />

§483.25(h) Accidents.<br />

The facility must ensure that –<br />

(1) The resident environment remains as free from accident hazards as is<br />

possible; <strong>and</strong><br />

(2) Each resident receives adequate supervision <strong>and</strong> assistance devices to<br />

prevent accidents.<br />

INTENT: 42 CFR 483.25(H) (1) AND (2) ACCIDENTS AND SUPERVISION<br />

The intent <strong>of</strong> this requirement is to ensure the facility provides an environment that is free<br />

from accident hazards over which the facility has control <strong>and</strong> provides supervision <strong>and</strong><br />

assistive devices to each resident to prevent avoidable accidents. This includes:<br />

Identifying hazard(s) <strong>and</strong> risk(s);
