o Pain;<br />

o Chronic psychiatric illness such as schizophrenia or schizoaffective<br />

disorder;<br />

o Acute psychotic illness such as brief reactive psychosis;<br />

o Substance intoxication or withdrawal;<br />

o Environmental stressors (e.g., excessive heat, noise, overcrowding);<br />

o Psychological stressors (e.g., disruption <strong>of</strong> the resident‘s customary daily<br />

routine, grief over nursing home admission or health status, abuse,<br />

taunting, intimidation);<br />

o Neurological illnesses such as Huntington‘s disease or Tourette‘s<br />

syndrome; or<br />

o Medical illnesses such as Alzheimer‘s disease, Lewy body disease,<br />

vascular dementia, or frontotemporal dementia.<br />

See Table I below in these guidelines for key issues related to indications for use <strong>of</strong><br />

antipsychotic agents, monitoring, <strong>and</strong> adverse consequences.<br />

II. Monitoring for Efficacy <strong>and</strong> Adverse Consequences<br />

The information gathered during the initial <strong>and</strong> ongoing evaluations is essential to:<br />

Incorporate into a comprehensive care plan that reflects appropriate medication<br />

related goals <strong>and</strong> parameters for monitoring the resident‘s condition, including the<br />

likely medication effects <strong>and</strong> potential for adverse consequences. Examples <strong>of</strong><br />

this information may include the FDA boxed warnings or adverse consequences<br />

that may be rare, but have sudden onset or that may be irreversible. If the facility<br />

has established protocols for monitoring specific medications <strong>and</strong> the protocols<br />

are accessible for staff use, the care plan may refer staff to these protocols;<br />

Optimize the therapeutic benefit <strong>of</strong> medication therapy <strong>and</strong> minimize or prevent<br />

potential adverse consequences;<br />

Establish parameters for evaluating the ongoing need for the medication; <strong>and</strong><br />

Verify or differentiate the underlying diagnoses or other underlying causes <strong>of</strong><br />

signs <strong>and</strong> symptoms.<br />

The key objectives for monitoring the use <strong>of</strong> medications are to track progress towards<br />

the therapeutic goal(s) <strong>and</strong> to detect the emergence or presence <strong>of</strong> any adverse<br />

consequences. Effective monitoring relies upon underst<strong>and</strong>ing the indications <strong>and</strong> goals<br />

for using the medication, identifying relevant baseline information, identifying the
