Multiple use items (e.g., shower chairs, bedside scales, resident lifts,<br />

commodes, tubs) are properly cleaned/disinfected between each resident<br />

use.<br />

Observe whether h<strong>and</strong> hygiene <strong>and</strong> use <strong>of</strong> gloves (when indicated) is in accordance with<br />

current st<strong>and</strong>ards. H<strong>and</strong> hygiene should occur before <strong>and</strong> after putting on sterile gloves<br />

<strong>and</strong> after taking <strong>of</strong>f all gloves during all resident care that requires the use <strong>of</strong> gloves. This<br />

includes:<br />

Medication administration (e.g., eye drops, sublinguals, <strong>and</strong> injections);<br />

Dressing changes that require the use <strong>of</strong> gloves (e.g., anticipated contact with<br />

body fluid, excretions, tissue <strong>and</strong> specimens);<br />

Insertion or removal <strong>of</strong> a catheter; <strong>and</strong><br />

Any invasive procedure.<br />

Note the availability <strong>of</strong> gloves <strong>and</strong> the equipment <strong>and</strong> products to perform h<strong>and</strong> hygiene.<br />

Interview<br />

During the resident review, interview the resident, family or responsible party to the<br />

extent possible to identify, as appropriate, whether they have received education <strong>and</strong><br />

information about infection control practices, such as appropriate h<strong>and</strong> hygiene <strong>and</strong> any<br />

special precautions applicable to the resident.<br />

Interview direct care staff to determine:<br />

Whether they are aware <strong>of</strong> <strong>and</strong> have reported any signs or symptoms exhibited by<br />

the resident that may be associated with an infection;<br />

Whether they are aware <strong>of</strong> <strong>and</strong> have been instructed on any special precautions<br />

that are applicable to any resident on transmission based precautions;<br />

Whether they are familiar with the indications for washing h<strong>and</strong>s <strong>and</strong>/or using<br />

alcohol based products <strong>and</strong> underst<strong>and</strong> the basis for the use <strong>of</strong> gloves <strong>and</strong> when<br />

they are to be removed;<br />

How staff know which residents are covered by transmission-based precautions;<br />

<strong>and</strong><br />

Whether staff is aware <strong>of</strong> what specific actions are required for each type <strong>of</strong><br />

transmission-based precautions.
