Beta adrenergic blockers,<br />

e.g.,<br />

Nonselective, e.g.,<br />

propranolol<br />

Cardioselective, e.g.,<br />

esmolol<br />

metoprolol<br />

nadolol<br />

timolol<br />

Calcium channel blockers,<br />

e.g.,<br />

methyldopa<br />

nifedipine<br />

isradipine<br />

amlodipine<br />

nisoldipine<br />

diltiazem<br />

verapamil<br />

Including combination<br />

products such as<br />

methyldopa/<br />

hydrochlorothiazide<br />

Potential for life-threatening elevation <strong>of</strong> serum<br />

potassium concentrations when used in<br />

combination with potassium supplements,<br />

potassium-sparing diuretics including<br />

spironolactone<br />

Adverse Consequences<br />

May cause or exacerbate:<br />

o Bradycardia, especially in individuals<br />

receiving other medications that affect<br />

cardiac conduction (e.g., calcium channel<br />

blockers);<br />

o Dizziness, fatigue; depression,<br />

bronchospasm (especially, but not<br />

exclusively, propranolol); or<br />

o Cardiac decompensation that may require<br />

adjusting dose in residents with acute heart<br />

failure<br />

May mask tachycardia associated with<br />

symptomatic hypoglycemia<br />

May have increased effect or may accumulate in<br />

individuals with hepatic impairment<br />

Adverse consequences<br />

May cause clinically significant constipation<br />

May cause peripheral edema<br />

Some agents may cause generalized aching,<br />

headache, muscle pain<br />

Short acting/immediate release nifedipine<br />

increases the risk <strong>of</strong> cardiac complications <strong>and</strong><br />

should not be used<br />

Indications<br />

Alternate treatments for hypertension are preferred<br />

Adverse Consequences<br />

May cause bradycardia <strong>and</strong> excessive sedation;<br />

may exacerbate depression in older individuals
