If the resident‘s PAS report indicates that he or she needs specialized services but the<br />

resident is not receiving them, notify the Medicaid agency. NF services ordinarily are not<br />

<strong>of</strong> the intensity to meet the needs <strong>of</strong> residents with MI or MR.<br />

Probes §483.20(m):<br />

If sampled residents have MI or MR, did the State Mental <strong>Health</strong> or Mental Retardation<br />

Authority determine:<br />

Whether the residents needed the services <strong>of</strong> a NF?<br />

Whether the residents need specialized services for their MR or MI?<br />

§483.25 Quality <strong>of</strong> Care<br />

Each resident must receive <strong>and</strong> the facility must provide the necessary care <strong>and</strong><br />

services to attain or maintain the highest practicable physical, mental, <strong>and</strong><br />

psychosocial well-being, in accordance with the comprehensive assessment <strong>and</strong> plan<br />

<strong>of</strong> care.<br />

Intent: §483.25<br />

The facility must ensure that the resident obtains optimal improvement or does not<br />

deteriorate within the limits <strong>of</strong> a resident‘s right to refuse treatment, <strong>and</strong> within the limits<br />

<strong>of</strong> recognized pathology <strong>and</strong> the normal aging process.<br />

NOTE: Use guidance at F309 for review <strong>of</strong> quality <strong>of</strong> care not specifically covered by<br />

42 CFR 483.25 (a)-(m). Tag F309 includes, but is not limited to, care such as<br />

end-<strong>of</strong>-life, diabetes, renal disease, fractures, congestive heart failure, nonpressure-related<br />

skin ulcers, pain, or fecal impaction.<br />

Definitions: §483.25<br />

―Highest practicable physical, mental, <strong>and</strong> psychosocial well-being‖ is defined as the<br />

highest possible level <strong>of</strong> functioning <strong>and</strong> well-being, limited by the individual‘s<br />

recognized pathology <strong>and</strong> normal aging process. Highest practicable is determined<br />

through the comprehensive resident assessment <strong>and</strong> by recognizing <strong>and</strong> competently<br />

<strong>and</strong> thoroughly addressing the physical, mental or psychosocial needs <strong>of</strong> the<br />

individual.<br />
