Antibiotics<br />

Medication Issues <strong>and</strong> Concerns<br />

All antibiotics Indications<br />

Parenteral vancomycin <strong>and</strong><br />

aminoglycosides, e.g.,<br />

amikacin<br />

gentamycin/<br />

gentamicin<br />

tobramycin<br />

consequences such as falls<br />

Use <strong>of</strong> antibiotics should be limited to confirmed<br />

or suspected bacterial infection<br />

Adverse Consequences<br />

Any antibiotic may cause diarrhea, nausea,<br />

vomiting, anorexia, <strong>and</strong> hypersensitivity/allergic<br />

reactions<br />

Antibiotics are non-selective <strong>and</strong> may result in the<br />

eradication <strong>of</strong> beneficial microorganisms <strong>and</strong> the<br />

emergence <strong>of</strong> undesired ones, causing secondary<br />

infections such as oral thrush, colitis, <strong>and</strong> vaginitis<br />

Monitoring<br />

Use must be accompanied by monitoring <strong>of</strong> renal<br />

function tests (which should be compared with<br />

the baseline) <strong>and</strong> by serum medication<br />

concentrations<br />

Serious adverse consequences may occur<br />

insidiously if adequate monitoring does not occur<br />

Exception: Single dose administration prophylaxis<br />

Adverse Consequences<br />

nitr<strong>of</strong>urantoin Indications<br />

Fluoroquinolones, e.g.,<br />

cipr<strong>of</strong>loxacin<br />

May cause or worsen hearing loss <strong>and</strong> renal<br />

failure<br />

It is not the anti-infective/antibiotic <strong>of</strong> choice for<br />

treatment <strong>of</strong> acute urinary tract infection or<br />

prophylaxis in individuals with impaired renal<br />

function (CrCl
