Patient-centered care should extend not only to the patient but to the family and<br />

caregivers <strong>of</strong> the patient. Patient-centeredness is one <strong>of</strong> the Institute <strong>of</strong> Medicine's<br />

(IOM's) aims for improvement in health care. In IOM's report "Crossing the Quality<br />

Chasm: A New Health System for the 21 st Century," providing patient-centered care is<br />

defined as "providing care that is respectful <strong>of</strong> and responsive to individual patient<br />

preferences, needs, and values, and ensuring that patient values guide all clinical<br />

) The National Partnership for Women and Families suggests the<br />

following principles for patient-centered care: (1) care is comprehensive, coordinated,<br />

personalized, and planned; (2) patients' experience <strong>of</strong> care is routinely assessed and<br />

improved; (3) patients and their caregivers are full partners in their care; (4) transitions<br />

between settings <strong>of</strong> care are smooth, safe, effective, and efficient; (5) patients can get<br />

care when and where they need it; (6) care is integrated with the community resources<br />

patients need to maintain health and wellbeing; and (7) continuous quality improvement<br />
